Indications
- Treatment of oral and gastrointestinal candidiasis
- Eradication of fungal colonization in the mouth and gastrointestinal tract
- Treatment of super infections due to gram-positive bacteria
- Indicated for dermatomycoses like tinea pedis, tinea corporis, tinea manuum, tinea cruris, candidial infections, tinea versicolor, infections of the skin, perianal infections, stomatitis angularis, balanoposthitis, otitis externa
- Rapid alleviating effect on pruritis
Pharmacology
- Antifungal activity against common dermatophytes and yeasts
- Antibacterial activity against certain gram-positive bacilli and cocci
- Inhibition of demethylation step in ergosterol biosynthesis
- Disruption of fungal cell membrane by disrupting ergosterol production
- Low oral bioavailability (25-30%)
- Binding to plasma proteins (88.2%) primarily to serum albumin and red blood cells (10.6%)
- Metabolism of absorbed portion
- Excretion of less than 1% unchanged in urine
- Terminal plasma half-life of 20-25 hours in most patients
- Similar elimination half-life in any renal impaired patient
Dosage
- Infants (4-24 months): 1.25 ml of gel, applied 4 times a day after meals
- Adults and children (2 years and older): 2.5 ml of gel, applied 4 times a day after meals
- Children and adults (4 months of age or above) with difficulty swallowing tablets: 20 mg per kg body weight per day, in four divided doses
- Missed dose instructions
- Application of sufficient cream to cover affected areas twice daily (morning and evening) for certain conditions
- Early relief of symptoms within 2-3 days for majority of patients
- Clinical improvement soon after treatment begins
- All lesions usually disappear within 2-5 weeks
- Prolonged treatment for 10 days to prevent relapse
- Varied treatment durations for different conditions
Administration
- Application 2-4 times a day directly to affected area with a clean finger for localized lesions of the mouth
- Recommended treatment duration of at least a week after symptoms disappear
- Removal and cleaning of dental prosthesis at night
Interaction
- Avoid concomitant treatment with Terfenadine, Astemizole, and Cisapride
- Potential interaction with Phenytoin and Cyclosporine
Contraindications
- Known hypersensitivity to the active ingredient
