Indications

Pharmacology
Metronidazole is a member of the imidazole class of antibacterial drug and is classified therapeutically as an antiprotozoal agent. The 5-nitro group of Metronidazole is reduced by anaerobes metabolically, and the reduced form of this drug interacts with DNA and gives bactericidal action of Metronidazole.
Dosage & Administration
- Tablet and Suspension:
- Various dosages for trichomoniasis in adults and children
- Dosages for intestinal amoebiasis in adults and children
- Dosages for extra-intestinal & asymptomatic amoebiasis in adults and children
- Dosages for giardiasis in adults and children
- Dosages for acute ulcerative gingivitis in adults and children
- Dosages for acute dental infections in adults and children
- Dosages for bacterial vaginosis in adults and children
- Dosages for leg ulcers and pressure sores in adults and children
- Dosages for anaerobic infections in adults and children
- Dosages for surgical prophylaxis in adults and children
- Vaginal Gel: Recommended dosage is one applicator full intravaginally once or twice a day for 5 days
- Suppository: Different dosages for anaerobic infections and surgical prophylaxis in adults and children
- IV Infusion:
- Adults and children over 12 years: Infuse 500 mg 8 hourly at a rate of 5 ml/minute, not to exceed 4 g in 24 hours
- Children under 12 years: 7.5 mg/kg body weight/day every 8 hours at a rate of 5 ml/minute
Interaction
- Disulfiram: May cause psychotic reactions when used concurrently with Vigil
- Alcohol: Should not be consumed during therapy and for at least one day afterwards
- Oral anticoagulant therapy: May potentiate the anticoagulant effect
- Lithium: May increase plasma levels of lithium
- Cyclosporin: Serum levels should be closely monitored when co-administered
- Phenytoin or phenobarbital: May increase elimination of Vigil
- 5-Fluorouracil: May reduce clearance of 5-fluorouracil
- Busulfan: May increase plasma levels of busulfan
Contraindications
Metronidazole is contraindicated in patients with a history of hypersensitivity to Metronidazole or other Nitroimidazole derivatives

- Regular hematological tests recommended for longer duration usage
- Caution in patients with hepatic encephalopathy
- May darken urine
Use in Special Populations
- Hepatic impairment: Dosage should be reduced and administered with caution
- Renal impairment: Dosage needs no reduction, metabolites are retained in patients
Overdose Effects
Symptomatic and supportive treatment recommended, no specific antidote available
