Indications

Pharmacology

  • Active against protozoa and anaerobic bacteria
  • Interacts with DNA to cause destruction of helical DNA structure
  • Inhibition of protein synthesis
  • Cell death in susceptible organisms

Dosage & Administration

  • Adult dosage for Amoebiasis: 500 mg twice a day for 5 days
  • Children dosage for Amoebiasis: 10-25 mg per kg body weight in two divided doses
  • Adult dosage for Amoebic dysentery: 1.5 gm once a day for 3 days
  • Children dosage for Amoebic dysentery: 40 mg per kg body weight, once a day for 3 days
  • Adult dosage for Giardiasis: 1.5 gm once daily for 1-2 days
  • Children dosage for Giardiasis: 40 mg per kg body weight for 2 days
  • Dosage for Trichomoniasis: 1.5 gm once or 500 mg twice a day for 5 days
  • Bacterial vaginosis dosage: 3 tablets of 500 mg each as a single dose or one tablet of 500 mg once daily for 5-7 days

Interaction

  • Mild potential to cause disulfiramlike reactions
  • May increase the risk of hemorrhage due to diminished hepatic metabolism when used with oral anticoagulants
  • Decreases the clearance of 5-fluorouracil

Contraindications

  • Previous hypersensitivity to Ornidazole and other nitroimidazoles
  • Central nervous system disorders, particularly in epilepsy or in peripheral neuropathy

Side Effects

  • Limited to the gastrointestinal tract (nausea, vomiting, epigastric pain)
  • Central nervous system effects (dizziness, headache, lassitude)
  • Leukopenia during therapy
  • Adverse CNS effects - headache, dizziness, lassitude or somnolence, fatigue, weakness

Pregnancy & Lactation

  • Should be prescribed only if the potential benefit justifies the potential risk to fetus/neonate

Precautions & Warnings

  • Prescribed with caution in patients with ataxia, vertigo, and mental confusion
  • Blood dyscrasia namely mild leukopenia has been reported rarely during prolonged treatment
  • Decision to discontinue therapy should depend upon the gravity of infection if leukopenia occurs

Use in Special Populations

  • Renal Impairment: Give a supplemental dose (50% of the usual dose) before dialysis in haemodialysis patients
  • Hepatic Impairment Severe: Double the interval between doses
