Interference with protein metabolism

Mode of Action

Inhibition of nucleic acid synthesis in malarial parasites

Dosage and Administration

  • Adult:
    • Oral: 600 mg (of quinine salt) every 8 hours for 7 days
    • Parenteral: Intravenous infusion - loading dose of 20 mg/kg (up to maximum 1.4 g) of quinine salt infused over 4 hours then after 8-12 hours maintenance dose of 10 mg/kg (up to maximum 700 mg) of quinine salt infused over 4 hours every 8-12 hours
  • Children:
    • Oral: 10 mg/kg (of quinine salt) every 8 hours for 7 days
    • Parenteral: Dosage calculated on a mg/kg basis
  • Pregnancy: Adult treatment doses of oral and intravenous quinine can safely be given to pregnant women

Contraindications

  • Hypersensitivity reaction
  • Optic neuritis
  • Black water fever

Side Effects

  • Cinchonism (tinnitus, headache, nausea, visual disturbances)
  • Hematological disorders
  • Skin reactions

Pregnancy and Lactation

Considered safe in therapeutic dosage during pregnancy, excreted in breast milk in insignificant amount

  • Severe heart disease
  • Myasthenia gravis
  • Diabetes mellitus
  • Slow IV injection
  • Increased chance of side effects when used with Mefloquine
