Indications

• Treatment of Plasmodium falciparum malaria
• Suppressive or prophylactic management

Pharmacology

• Antimalarial agent with synergistic effect
• Effective against resistant strains
• Long-acting and attacks different parasite development stages

Dosage & Administration

• Curative treatment dosage for adults and children
• Combination with quinine for severe cases
• Different dosages for semi-immune and non-immune subjects
• Malaria prophylaxis dosage

Interaction

• Concurrent administration with folate antagonists can lead to haematological side effects

Contraindications

• Contraindicated in severe renal insufficiency
• Contraindicated in marked liver parenchymal damage
• Contraindicated in blood dyscrasias
• Immediate discontinuation upon appearance of mucocutaneous signs or symptoms

Side Effects

• Well tolerated at recommended dose
• Skin reactions such as drug rash and pruritus
• Gastro-intestinal reactions like nausea and vomiting
• Rare haematological reactions
• Other reactions like fatigue and headache

Pregnancy & Lactation

• Contraindicated during pregnancy and lactation
• Risks should be balanced against therapeutic effect
• Excretion in maternal breast milk

Precautions & Warnings

• Impaired renal or hepatic function
• Folate deficiency
• Severe allergy or bronchial asthma
• G6PD deficiency
• Discontinue at first sign of rash
• Regular CBC monitoring and LFT analysis
• Take with plenty of water to prevent crystalluria
• Avoid excessive sun exposure
