Description

Management Indications

  • Management of hypertension alone or concomitantly with other antihypertensive agents
  • Long-term management of patients with angina pectoris
  • Management of hemodynamically stable patients with definite or suspected acute myocardial infarction to reduce cardiovascular mortality

Mode of Action

  • Selectively blocks beta1 (cardiac) receptors
  • Highly hydrophilic with poor penetration of lipid cell membranes

Dosage & Administration

  • Initial dose of 50 mg once daily for hypertension
  • Initial dose of 50 mg once daily for angina pectoris
  • Initial intravenous dose of 5 mg followed by oral doses for acute myocardial infarction

Interactions

  • Catecholamine-depleting drugs may have an additive effect
  • Calcium channel blockers may have an additive effect
  • Disopyramide and amiodarone may have additive effects
  • Exacerbates rebound hypertension when coadministered with clonidine
  • Concomitant use with prostaglandin synthase inhibiting drugs may decrease hypotensive effects
  • Potential for more severe allergic reaction in patients with a history of anaphylaxis
  • Potential for increased risk of bradycardia when coadministered with digitalis glycosides

Contraindications

  • Sinus bradycardia, heart block greater than first degree, cardiogenic shock, overt cardiac failure
  • Hypersensitivity to atenolol or any of the drug's components

Side Effects

  • Agranulocytosis
  • Fever, sore throat, laryngospasm, respiratory distress
  • Reversible mental depression, short term memory loss
  • Mesenteric arterial thrombosis, ischemic colitis
  • Skin rashes, dry eyes
  • Erythematous rash

Pregnancy & Lactation

Pregnancy Category D, caution in nursing mothers

Precautions & Warnings

  • Careful evaluation for patients already on beta blockers before administration
  • Aggravation of peripheral arterial circulatory disorders
  • Caution in impaired renal function
  • Caution and cautious dosing in geriatric patients with coronary atherosclerosis or acute myocardial infarction, evaluation of renal function for hypertension or myocardial infarction patients

Use in Special Populations

Dosage adjustment for elderly and renal-impaired patients based on renal function

Overdose Effects

Lethargy, respiratory disorders, wheezing, bradycardia, congestive heart failure, hypotension, bronchospasm, hypoglycemia
