Pharmacology

  • Combination of Paracetamol and Caffeine
  • Analgesic and antipyretic properties of Paracetamol
  • Weak anti-inflammatory activity of Paracetamol
  • Caffeine as an alkaloid and theophylline-like xanthine derivative
  • Increased solubility and transmembrane permeation of Paracetamol due to intermolecular association with Caffeine
  • Increased pain threshold and tolerance of pain
  • Intrinsic power of Caffeine to raise vessel tone in the brain

Dosage & Administration

  • Adult dose: 1-2 tablets every 4-6 hours
  • Maximum adult dose: 8 tablets daily
  • Not recommended for children below 12 years

Interaction

  • May reduce serum levels with anticonvulsants
  • Enhanced anticoagulant effect of warfarin and other coumarins with prolonged use
  • Accelerated absorption with metoclopramide and domperidone
  • Increased serum levels with probenecid
  • Increased serum levels of chloramphenicol
  • Reduced absorption with colestyramine within 1 hr of admin
  • Severe hypothermia with phenothiazine

Contraindications

  • Severe renal function impairment
  • Hepatic disease (Viral Hepatitis)
  • Known hypersensitivity to Paracetamol or Caffeine

Side Effects

  • Mild side effects of Paracetamol
  • Haematological reactions including thrombocytopenia, leukopenia, pancytopenia, neutropenia, and agranulocytosis
  • Pancreatitis, skin rashes, and other allergic reactions

Pregnancy & Lactation

  • Consultation required for pregnant mothers
  • Paracetamol & Caffeine safe for breastfeeding

Precautions & Warnings

  • Cautious use in patients with hepatic or renal failure
  • Patients taking other hepatotoxic medication
  • Avoid prolonged use without consulting a physician

Overdose Effects

  • Symptoms of Paracetamol overdose in the first 24 hours like pallor, nausea, vomiting, anorexia, and abdominal pain
  • Liver damage may appear 12 to 40 hours after ingestion
  • Abnormalities of glucose metabolism and metabolic acidosis may occur
