Indications
- Treatment of moderate to severe painful conditions
- Postoperative pain
- Colic and spastic pain

Pharmacology
- Centrally acting synthetic analgesic compound
- Inhibits reuptake of neurotransmitters- serotonin and noradrenaline
- Modifies transmission of pain impulses
- Activates descending serotonergic pathways
- Activates noradrenergic pathways involved in analgesia
- Mediated via stimulation of mu-opioid receptors
- Indirect modulation of central monoaminergic inhibitory pathways
Dosage & Administration
- Usual doses of 50 to 100 mg every four to six hours
- Initial dose of 100 mg for acute pain
- Initial dose of 50 mg for chronic painful conditions
- Subsequent doses of 50 to 100 mg every 4-6 hourly
- Total daily dosage by mouth not to exceed 400 mg
- Sustained release capsule to be taken every 12 hours
- Injection dose of 50-100 mg every 4 to 6 hours
- Rectal suppository dose of 100 mg Tramadol Hydrochloride 6 hourly
- Total daily suppository dosage not to exceed 400 mg
- Higher daily doses for cancer pain and severe pain after operations
Interaction
- No concern about most drugs interacting with Anadol
- Avoid combination with monoamine oxidase (MAO) inhibitors
- Concomitant administration of carbamazepine can increase Anadol metabolism
Contraindications
- Hypersensitivity to Tramadol
- Contraindicated in acute intoxication with alcohol, hypnotics, centrally acting analgesics, opioids or psychotropic drugs

Pregnancy & Lactation
- Safe use in pregnancy not established
- Tramadol crosses the placenta
- No adequate and well-controlled studies in pregnant women
- Use during pregnancy only if potential benefit justifies risk to the fetus
- Not to be administered during breastfeeding as Tramadol and its metabolites detected in breast milk
Precautions & Warnings
- Cautious administration in patients at risk for respiratory depression
- Not recommended for patients dependent on opioids
- Use with caution and in reduced dosages when administering with CNS depressants
- Use with great caution in patients taking MAO inhibitors
- Use with caution in patients with increased intracranial pressure or head injury
- Use with caution in patients with acute abdominal conditions
Use in Special Populations
- In children from the age of 1 year, Anadol can be given in a dose of 1-2 mg/kg body weight
- Suppository (100 mg Anadol) not to be administered in children and adolescents below the age of 14 years
- Anadol 100 mg SR Capsules not studied in children, safety and efficacy not established
