Indications
- Treatment of moderate to severe painful conditions
- Postoperative pain
- Colic and spastic pain

Pharmacology
- Centrally acting synthetic analgesic compound
- Inhibition of reuptake of neurotransmitters- serotonin and noradrenaline
- Modifies the transmission of pain impulses
- Stimulation of mu-opioid receptors
- Modulation of central monoaminergic inhibitory pathways
Dosage & Administration
- Usual doses of 50 to 100 mg every four to six hours
- Initial dose of 100 mg for acute pain
- Initial dose of 50 mg for chronic painful conditions
- Subsequent doses of 50 to 100 mg administered 4-6 hourly
- Total daily dosage by mouth should not exceed 400 mg
- One sustained release capsule or tablet every 12 hours
- 50-100 mg injection every 4 to 6 hours
- Tramadol suppository administered rectally, with usual dose of 100 mg every 6 hours for adults
Interaction
- Monoamine oxidase (MAO) inhibitors not recommended for combination with Doloran
- Concomitant administration of carbamazepine requires adjustment of Doloran dose
Contraindications
- Hypersensitivity to Tramadol
- Acute intoxication with alcohol, hypnotics, opioids or psychotropic drugs
Side Effects
- Common side effects: dizziness/vertigo, nausea, constipation, headache, somnolence, vomiting, pruritus, CNS stimulation, asthenia, sweating, dyspepsia, dry mouth, diarrhoea
- Less common side effects: malaise, allergic reaction, weight loss, vasodilatation, palpitations, abdominal pain, anorexia, flatulence, GI bleeding, hepatitis, stomatitis
Pregnancy & Lactation
- Safe use in pregnancy not established
- Crosses the placenta
- Use during pregnancy only if potential benefit justifies risk to the fetus
- Not recommended during breastfeeding
Precautions & Warnings
- Cautious administration in patients at risk for respiratory depression
- Not recommended for patients dependent on opioids
- Caution with concomitant CNS depressants
- Caution in patients taking MAO inhibitors
- Use with caution in patients with increased intracranial pressure or head injury and acute abdominal conditions
Use in Special Populations
- Dose of 1-2 mg/kg body weight in children from the age of 1 year
- Suppository not for administration in children and adolescents below the age of 14 years
- Doloran 100 mg SR Capsules not studied in children; safety and efficacy not established
