Indications
- Treatment of moderate to severe painful conditions
- Postoperative pain
- Colic and spastic pain
- Cancer pain
- Joint pain
- Neck and back pain
- Pain associated with osteoporosis
Pharmacology
- Centrally acting synthetic analgesic compound
- Inhibition of reuptake of neurotransmitters- serotonin and noradrenaline
- Modification of transmission of pain impulses
- Activation of descending serotonergic pathways and noradrenergic pathways
- Stimulation of mu-opioid receptors
- Modulation of central monoaminergic inhibitory pathways
Dosage & Administration
- Usual doses of 50 to 100 mg every four to six hours
- Initial dose of 100 mg for acute pain
- Initial dose of 50 mg for chronic painful conditions
- Total daily dosage by mouth should not exceed 400 mg
- Sustained release capsule or tablet dosage every 12 hours
- Injection dosage of 50-100 mg every 4 to 6 hours
- Suppository dosage of 100 mg Tramadol Hydrochloride 6 hourly for adults
Interaction
- Monoamine oxidase (MAO) inhibitors are contraindicated
- Concomitant administration of carbamazepine increases Dolorex metabolism
Contraindications
- Hypersensitivity to Tramadol
- Acute intoxication with alcohol, hypnotics, centrally acting analgesics, opioids or psychotropic drugs

Pregnancy & Lactation
- Safe use in pregnancy not established
- Tramadol crosses the placenta
- Potential benefit justifies the risk to the fetus
- Tramadol and its metabolites detected in breast milk
Precautions & Warnings
- Respiratory depression risk
- Not recommended for opioid-dependent patients
- Use with caution with concomitant CNS depressants
- Great caution in patients taking MAO inhibitors
- Use with caution in patients with increased intracranial pressure or head injury and acute abdominal conditions
Use in Special Populations
- Dolorex can be given in children from the age of 1 year in specified doses
- Suppository (100 mg Dolorex) should not be administered in children and adolescents below the age of 14 years
- Dolorex 100 mg SR Capsules have not been studied in children
