Indications
- Moderate to severe painful conditions
- Postoperative pain
- Colic and spastic pain

Pharmacology
Centrally acting synthetic analgesic compound; inhibits reuptake of neurotransmitters serotonin and noradrenaline; modifies transmission of pain impulses by activating serotonergic and noradrenergic pathways involved in analgesia; mediated via stimulation of mu-opioid receptors and indirect modulation of central monoaminergic inhibitory pathways
Dosage & Administration
- Usual doses 50-100 mg every 4-6 hours
- Initial dose 100 mg for acute pain, 50 mg for chronic painful conditions
- Sustained release capsules 1 every 12 hours
- Injection 50-100 mg every 4-6 hours; 100 mg initial dose for postoperative pain
- Suppository 100 mg every 6 hours; higher doses for cancer pain and severe pain after operations; maximum daily dose 400 mg
Interactions
- Monoamine oxidase (MAO) inhibitors are not recommended for combination
- Concomitant administration of carbamazepine increases metabolism of Retram, requiring dose adjustment
Contraindications
- Hypersensitivity to tramadol
- Acute intoxication with alcohol, hypnotics, centrally acting analgesics, opioids, or psychotropic drugs

Precautions & Warnings
- Respiratory depression risk with large doses
- Not recommended for patients dependent on opioids
- Caution with CNS depressants
- Caution with MAO inhibitors
- Caution in patients with increased intracranial pressure or head injury, acute abdominal conditions
Use in Special Populations
- 1-2 mg/kg body weight in children age 1+
- Suppository not for children below 14 years
- 100 mg SR Capsules not studied in children
