Indications

  • Treatment of moderate to severe painful conditions
  • Postoperative pain
  • Colic and spastic pain
  • Cancer pain
  • Joint pain
  • Neck and back pain
  • Pain associated with osteoporosis

Pharmacology

  • Centrally acting synthetic analgesic
  • Inhibits reuptake of serotonin and noradrenaline
  • Modifies transmission of pain impulses
  • Stimulation of mu-opioid receptors
  • Modulation of central monoaminergic pathways

Dosage & Administration

  • Usual doses of 50 to 100 mg every 4-6 hours
  • Initial dose of 100 mg for acute pain
  • Initial dose of 50 mg for chronic painful conditions
  • Sustained release capsule for every 12 hours
  • Injection dose of 50-100 mg every 4-6 hours
  • Suppository dose of 100 mg every 6 hours

Interaction

  • Contraindicated with monoamine oxidase (MAO) inhibitors
  • Increased metabolism with concomitant carbamazepine
  • No general concern about drug interactions

Contraindications

  • Hypersensitivity to tramadol
  • Acute intoxication with certain substances

Side Effects

  • Common side effects such as dizziness, nausea, constipation, headache, somnolence
  • Less common side effects including malaise, allergic reaction, weight loss, palpitations, etc.

Pregnancy & Lactation

  • Safety not established in pregnancy
  • Crosses placenta
  • Not recommended during breastfeeding

Precautions & Warnings

  • Caution in patients at risk for respiratory depression
  • Not recommended for opioid-dependent patients
  • Reduced dosages when using with CNS depressants
  • Great caution when using with MAO inhibitors
  • Caution in patients with increased intracranial pressure or head injury, acute abdominal conditions

Use in Special Populations

  • Can be given to children from the age of 1 year
  • Suppository not for use in children and adolescents below the age of 14 years
  • Safety and efficacy not established for children with 100 mg SR Capsules
