Description
- Utramal is a centrally acting synthetic analgesic compound used for the treatment of moderate to severe painful conditions such as postoperative pain, colic and spastic pain, cancer pain, joint pain, neck and back pain, and pain associated with osteoporosis.
Indications
- Moderate to severe painful conditions
- Postoperative pain relief
- Colic and spastic pain relief
- Cancer pain relief
- Joint pain relief
- Neck and back pain relief
- Pain relief associated with osteoporosis
Pharmacology
- Centrally acting synthetic analgesic
- Inhibits reuptake of neurotransmitters
- Modifies transmission of pain impulses
- Stimulation of mu-opioid receptors
- Modulation of central monoaminergic inhibitory pathways
Dosage and administration
- Usual doses of 50 to 100 mg every four to six hours
- Initial dose of 100 mg for acute pain
- Initial dose of 50 mg for chronic painful conditions
- Total daily dosage by mouth should not exceed 400 mg
- Sustained release capsule or tablet dosage
- Injection dosage
- Suppository dosage
Interaction
- Non-recommendation for combination with monoamine oxidase (MAO) inhibitors
- Significant increase in metabolism with concomitant administration of carbamazepine
Contraindications
- Hypersensitivity to tramadol
- Acute intoxication with alcohol, hypnotics, centrally acting analgesics, opioids or psychotropic drugs
Side effects
- Common side-effects like dizziness, nausea, constipation, headache, somnolence, vomiting
- Less common side-effects like allergic reaction, weight loss, vasodilatation, palpitations, GI bleeding, hepatitis
Pregnancy and lactation
- Safe use not established in pregnancy
- Tramadol crosses the placenta
- Risk to the fetus should be justified by potential benefit
- Avoid use during breastfeeding
Precautions and warnings
- Respiratory depression risk
- Not recommended for opioid-dependent patients
- Cautious use with CNS depressants and MAO inhibitors
- Caution in patients with increased intracranial pressure, head injury, and acute abdominal conditions
Use in special populations
- Dosage for children from the age of 1 year
- Avoid suppository in children and adolescents below the age of 14 years
- Safety and efficacy of Utramal 100 mg SR Capsules not established in children
