Indications
- Management of hypertension
- Long-term management of angina pectoris
- Management of hemodynamically stable patients with definite or suspected acute myocardial infarction
Pharmacology
- Beta-1 selective beta-blocker
- Highly hydrophilic
- Competitively blocks beta-1 (cardiac) receptors
Dosage & Administration
- Initial dose for hypertension: 50mg/day
- Initial dose for angina pectoris: 50mg/day
- Initial dose for acute myocardial infarction: 5mg intravenous injection
Interaction
- Additive effect with catecholamine-depleting drugs
- Additive effect with calcium channel blockers
- Potent negative inotropic and chronotropic effects when administered with disopyramide
- Additive negative chronotropic properties when administered with amiodarone
Contraindications
- Sinus bradycardia
- Greater than first degree heart block
- Cardiogenic shock
Side Effects
- Agranulocytosis
- Fever, sore throat, respiratory distress
- Reversible mental depression
- Mesenteric arterial thrombosis
- Skin rashes and dry eyes
Pregnancy & Lactation
- Pregnancy Category D
- Caution during nursing
- Clinically significant bradycardia reported in breast-fed infants
Precautions & Warnings
- Aggravates peripheral arterial circulatory disorders
- Use with caution in impaired renal function
Use in Special Populations
- Dosage adjustment for elderly patients
- Maximum dosage limits based on renal function
- Lower starting dose for some patients with hypertension
Overdose Effects
- Reported overdoses survived up to 5g
- Common effects include lethargy, respiratory disorders, bradycardia
- Possible congestive heart failure, hypotension, bronchospasm, hypoglycemia
