Indications
- Moderate to severe painful conditions
- Postoperative pain
- Colic and spastic pain

Pharmacology
- Centrally acting synthetic analgesic compound
- Inhibit re-uptake of neurotransmitters
- Modifies transmission of pain impulses
- Stimulation of mu-opioid receptors
- Modulation of central monoaminergic inhibitory pathways
Dosage & Administration
- Usual doses of 50 to 100 mg every 4-6 hours
- Initial dose of 100 mg for acute pain
- Initial dose of 50 mg for chronic painful conditions
- Total daily dosage by mouth should not exceed 400 mg
- Sustained release capsule or tablet for 12-hour relief
- Injection dosage of 50-100 mg every 4-6 hours
- Maximum total daily dose of 600 mg for postoperative pain
- Rectal suppository administration: usual dose of 100 mg every 6 hours

Contraindications with monoamine oxidase (MAO) inhibitors

- Acute intoxication with alcohol, hypnotics, opioids, or psychotropic drugs

Precautions & Warnings
- Cautious administration in patients at risk for respiratory depression
- Not recommended for opioid-dependent patients
- Caution with concomitant CNS depressants
- Great caution with concomitant MAO inhibitors
- Caution in patients with increased intracranial pressure or head injury
- Caution in patients with acute abdominal conditions
Use in Special Populations
- Can be given in children from the age of 1 year
- Suppository should not be given to children and adolescents below 14 years
- Tranal 100 mg SR Capsules not studied in children
