Indications
- Treatment of moderate to severe painful conditions
- Postoperative pain
- Colic and spastic pain
- Cancer pain
- Joint pain
- Neck and back pain
- Pain associated with osteoporosis
Pharmacology
- Centrally acting synthetic analgesic compound
- Inhibits reuptake of neurotransmitters (serotonin and noradrenaline)
- Modifies transmission of pain impulses
- Activates descending serotonergic pathways
- Activates noradrenergic pathways involved in analgesia
- Effects mediated via stimulation of mu-opioid receptors
- Indirect modulation of central monoaminergic inhibitory pathways
Dosage & Administration
- Usual doses of 50 to 100 mg every four to six hours
- Initial doses of 100 mg for acute pain
- Initial doses of 50 mg for chronic painful conditions
- Sustained release capsule or tablet for long-lasting relief
- Injection for rapid onset of action
- Suppository for rectal administration
Interactions
- No significant drug interactions except with monoamine oxidase (MAO) inhibitors
- Carbamazepine can increase metabolism of Tramadol
Contraindications
- Hypersensitivity to Tramadol
- Acute intoxication with alcohol, hypnotics, centrally acting analgesics, opioids, or psychotropic drugs
Side Effects
- Common side effects: dizziness, nausea, constipation, headache, somnolence, vomiting
- Less common side effects: malaise, allergic reaction, weight loss, abdominal pain, GI bleeding
Pregnancy & Lactation
- Safety in pregnancy not established
- Crosses placenta
- Use during pregnancy only if potential benefit justifies risk to fetus
- Not to be used during breastfeeding
Precautions & Warnings
- Caution in patients at risk for respiratory depression
- Not recommended for patients dependent on opioids
- Use with caution in patients receiving CNS depressants
- Use with caution in patients taking MAO inhibitors
- Caution in patients with increased intracranial pressure, head injury, or acute abdominal conditions
Use in Special Populations
- Safe use in children from age 1 year at 1-2 mg/kg body weight
- Suppository should not be administered in children below age 14 years
- SR Capsules not studied in children
