Indications

  • Treatment of moderate to severe painful conditions
  • Postoperative pain
  • Colic and spastic pain
  • Cancer pain
  • Joint pain
  • Neck and back pain
  • Pain associated with osteoporosis

Pharmacology

  • Centrally acting synthetic analgesic compound
  • Inhibits reuptake of neurotransmitters (serotonin and noradrenaline)
  • Modifies transmission of pain impulses
  • Activates descending serotonergic pathways
  • Activates noradrenergic pathways involved in analgesia
  • Effects mediated via stimulation of mu-opioid receptors
  • Indirect modulation of central monoaminergic inhibitory pathways

Dosage & Administration

  • Usual doses of 50 to 100 mg every four to six hours
  • Initial doses of 100 mg for acute pain
  • Initial doses of 50 mg for chronic painful conditions
  • Sustained release capsule or tablet for long-lasting relief
  • Injection for rapid onset of action
  • Suppository for rectal administration

Interactions

  • No significant drug interactions except with monoamine oxidase (MAO) inhibitors
  • Carbamazepine can increase metabolism of Tramadol

Contraindications

  • Hypersensitivity to Tramadol
  • Acute intoxication with alcohol, hypnotics, centrally acting analgesics, opioids, or psychotropic drugs

Side Effects

  • Common side effects: dizziness, nausea, constipation, headache, somnolence, vomiting
  • Less common side effects: malaise, allergic reaction, weight loss, abdominal pain, GI bleeding

Pregnancy & Lactation

  • Safety in pregnancy not established
  • Crosses placenta
  • Use during pregnancy only if potential benefit justifies risk to fetus
  • Not to be used during breastfeeding

Precautions & Warnings

  • Caution in patients at risk for respiratory depression
  • Not recommended for patients dependent on opioids
  • Use with caution in patients receiving CNS depressants
  • Use with caution in patients taking MAO inhibitors
  • Caution in patients with increased intracranial pressure, head injury, or acute abdominal conditions

Use in Special Populations

  • Safe use in children from age 1 year at 1-2 mg/kg body weight
  • Suppository should not be administered in children below age 14 years
  • SR Capsules not studied in children
