Indications
- Relief of signs and symptoms of osteoarthritis
- Relief of signs and symptoms of rheumatoid arthritis
- Relief of signs and symptoms of ankylosing spondylitis
- Decrease the risk of developing gastric ulcers in patients at risk of developing NSAID-associated gastric ulcers
Pharmacology
- Immediate release Esomeprazole Magnesium layer
- Enteric-coated Naproxen core
- Mechanism of action of Naproxen to inhibit prostaglandin synthesis
- Mechanism of action of Esomeprazole as a proton pump inhibitor to suppress gastric acid secretion
Dosage
- Use lowest effective dose for the shortest duration consistent with individual patient treatment goals
- Adult dosage: One tablet twice daily of either 375 mg naproxen/20 mg esomeprazole or 500 mg naproxen/20 mg esomeprazole
- Juvenile Idiopathic Arthritis dosage: Varies based on patient weight
Administration
- Tablet to be taken at least 30 minutes before meals
- Do not split, chew, crush, or dissolve the tablet
Interaction
- Concomitant use of NSAIDs may reduce the antihypertensive effect of ACE inhibitors, diuretics & beta-blockers
- Concomitant use of tablet and warfarin may result in an increased risk of a bleeding complication
- Esomeprazole may interfere with the absorption of drugs where gastric pH is an important determinant of bioavailability
- Administration of tablet together with high-fat food in healthy volunteers does not affect the extent of absorption of naproxen
Contraindications
- Known hypersensitivity to any component of this tablet or substituted benzimidazoles
- History of asthmay urticaria or other allergic-type reactions after taking aspirin or other NSAIDs
- Use during the peri-operative period in the setting of coronary artery bypass graft (CABG) surgery
Side Effects
- Decrease in occurrence of gastric ulcers and NSAID associated upper gastrointestinal adverse events compared to Naproxen alone
- Gastrointestinal adverse events, arterial thrombotic events, oedema, hypertension, cardiac failure
- Commonly observed adverse events are gastrointestinal in nature
- Potential reduction in absorption of vitamin B12 due to hypo- or achlorhydria induced by acid-blocking medicines
Pregnancy & Lactation
- Contraindicated during the third trimester of pregnancy
- Not recommended during breastfeeding
- Potential impairment of female fertility with NSAID use
Precautions & Warnings
- Avoid the combination of Naproxen and NSAIDs including cyclooxygenase-2 selective inhibitors
- Consideration for patients with risk factors for cardiovascular events
- Regular surveillance for patients on long-term treatment over a year
Use in Special Populations
- Dosage adjustment for elderly patients
- Not recommended in patients with moderate to severe or severe renal impairment
- Close monitoring for patients with mild to moderate hepatic impairment
Overdose Effects
- Symptoms related to Naproxen overdose including lethargy, dizziness, epigastric pain, abdominal discomfort, heartburn, nausea, alterations in liver function
- Management includes symptomatic and supportive care, emesis, activated charcoal, and osmotic cathartic
