Indications
- Treatment of moderate to severe painful conditions
- Postoperative pain
- Colic and spastic pain
- Cancer pain
- Joint pain
- Neck and back pain
- Pain associated with osteoporosis
Pharmacology
- Centrally acting synthetic analgesic compound
- Inhibits reuptake of neurotransmitters
- Modifies transmission of pain impulses
- Activates descending serotonergic pathways
- Activates noradrenergic pathways
- Stimulation of mu-opioid receptors
- Modulation of central monoaminergic inhibitory pathways
Dosage & Administration
- Usual doses of capsule or tablet: 50 to 100 mg every 4-6 hours
- Initial dose for acute pain: 100 mg
- Initial dose for chronic pain: 50 mg
- Total daily dosage by mouth should not exceed 400 mg
- Sustained release capsule or tablet: One every 12 hours
- Maximum total daily dosage for sustained release: 400 mg
- Injection dose: 50-100 mg every 4-6 hours
- Maximum total daily dosage for injection: 600 mg
- Suppository: 100 mg every 6 hours
- High daily doses for cancer pain and severe pain after operations
Interaction
- Avoid monoamine oxidase (MAO) inhibitors
Contraindications
- Hypersensitivity to Tramadol
- Acute intoxication with alcohol or certain drugs

Pregnancy & Lactation
- Not established as safe for use in pregnancy
- Tramadol crosses the placenta
- Potential benefit justified the risk to the fetus
- Not recommended during breastfeeding
Precautions & Warnings
- Risk of respiratory depression
- Not recommended for patients dependent on opioids
- Caution with concurrent CNS depressants
- Caution with concurrent MAO inhibitors
- Caution in patients with increased intracranial pressure
- Caution in patients with acute abdominal conditions
Use in Special Populations
- Children from the age of 1 year can be given Winpain
- Suppository not recommended below the age of 14 years
- 100 mg SR Capsules not studied in children
