Indications

  • Treatment of moderate to severe painful conditions
  • Postoperative pain
  • Colic and spastic pain
  • Cancer pain
  • Joint pain
  • Neck and back pain
  • Pain associated with osteoporosis

Pharmacology

  • Centrally acting synthetic analgesic compound
  • Inhibits reuptake of neurotransmitters
  • Modifies transmission of pain impulses
  • Activates descending serotonergic pathways
  • Activates noradrenergic pathways
  • Stimulation of mu-opioid receptors
  • Modulation of central monoaminergic inhibitory pathways

Dosage & Administration

  • Usual doses of capsule or tablet: 50 to 100 mg every 4-6 hours
  • Initial dose for acute pain: 100 mg
  • Initial dose for chronic pain: 50 mg
  • Total daily dosage by mouth should not exceed 400 mg
  • Sustained release capsule or tablet: One every 12 hours
  • Maximum total daily dosage for sustained release: 400 mg
  • Injection dose: 50-100 mg every 4-6 hours
  • Maximum total daily dosage for injection: 600 mg
  • Suppository: 100 mg every 6 hours
  • High daily doses for cancer pain and severe pain after operations

Interaction

  • Avoid monoamine oxidase (MAO) inhibitors

Contraindications

  • Hypersensitivity to Tramadol
  • Acute intoxication with alcohol or certain drugs

Pregnancy & Lactation

  • Not established as safe for use in pregnancy
  • Tramadol crosses the placenta
  • Potential benefit justified the risk to the fetus
  • Not recommended during breastfeeding

Precautions & Warnings

  • Risk of respiratory depression
  • Not recommended for patients dependent on opioids
  • Caution with concurrent CNS depressants
  • Caution with concurrent MAO inhibitors
  • Caution in patients with increased intracranial pressure
  • Caution in patients with acute abdominal conditions

Use in Special Populations

  • Children from the age of 1 year can be given Winpain
  • Suppository not recommended below the age of 14 years
  • 100 mg SR Capsules not studied in children
