Indications

  • Treatment of moderate to severe painful conditions
  • Postoperative pain
  • Colic and spastic pain
  • Cancer pain
  • Joint pain
  • Neck and back pain
  • Pain associated with osteoporosis

Pharmacology

  • Centrally acting synthetic analgesic compound
  • Inhibits reuptake of neurotransmitters (serotonin and noradrenaline)
  • Modifies transmission of pain impulses
  • Stimulation of mu-opioid receptors
  • Indirect modulation of central monoaminergic inhibitory pathways

Dosage & Administration

  • Usual doses are 50 to 100 mg every four to six hours
  • Initial dose of 100 mg for acute pain
  • Initial dose of 50 mg for chronic painful conditions
  • Sustained release capsule: one SR capsule every 12 hours
  • Injection: 50-100 mg every 4 to 6 hours
  • Suppository: 100 mg Tramadol Hydrochloride 6 hourly for adults

Interaction

  • Monoamine oxidase (MAO) inhibitors are not recommended for combination
  • Concomitant administration of carbamazepine increases Xtrapel metabolism

Contraindications

  • Hypersensitivity to the drug
  • Acute intoxication with alcohol, hypnotics, centrally acting analgesics, opioids, or psychotropic drugs

Pregnancy & Lactation

  • Safe use not established in pregnancy
  • Crosses the placenta
  • Should not be administered during breastfeeding

Precautions & Warnings

  • Respiratory depression with large doses
  • Not recommended for opioid-dependent patients
  • Use with caution when administering with CNS depressants
  • Use with caution in patients taking MAO inhibitors
  • Use with caution in patients with increased intracranial pressure or head injury
  • Use with caution in patients with acute abdominal conditions

Use in Special Populations

  • Can be given in children from the age of 1 year in a specific dose
  • Suppository should not be administered in children and adolescents below the age of 14 years
  • Xtrapel 100 mg SR Capsules not studied in children
