Indications
- Treatment of moderate to severe painful conditions
- Postoperative pain
- Colic and spastic pain
- Cancer pain
- Joint pain
- Neck and back pain
- Pain associated with osteoporosis
Pharmacology
- Centrally acting synthetic analgesic compound
- Inhibits reuptake of neurotransmitters (serotonin and noradrenaline)
- Modifies transmission of pain impulses
- Stimulation of mu-opioid receptors
- Indirect modulation of central monoaminergic inhibitory pathways
Dosage & Administration
- Usual doses are 50 to 100 mg every four to six hours
- Initial dose of 100 mg for acute pain
- Initial dose of 50 mg for chronic painful conditions
- Sustained release capsule: one SR capsule every 12 hours
- Injection: 50-100 mg every 4 to 6 hours
- Suppository: 100 mg Tramadol Hydrochloride 6 hourly for adults
Interaction
- Monoamine oxidase (MAO) inhibitors are not recommended for combination
- Concomitant administration of carbamazepine increases Xtrapel metabolism
Contraindications
- Hypersensitivity to the drug
- Acute intoxication with alcohol, hypnotics, centrally acting analgesics, opioids, or psychotropic drugs

Pregnancy & Lactation
- Safe use not established in pregnancy

Precautions & Warnings
- Respiratory depression with large doses
- Not recommended for opioid-dependent patients
- Use with caution when administering with CNS depressants
- Use with caution in patients taking MAO inhibitors
- Use with caution in patients with increased intracranial pressure or head injury
- Use with caution in patients with acute abdominal conditions
Use in Special Populations
- Can be given in children from the age of 1 year in a specific dose
- Suppository should not be administered in children and adolescents below the age of 14 years
- Xtrapel 100 mg SR Capsules not studied in children
