Indications
- Treatment of moderate to severe painful conditions
- Postoperative pain
- Colic and spastic pain
- Cancer pain
- Joint pain
- Neck and back pain
- Pain associated with osteoporosis
Pharmacology
- Centrally acting synthetic analgesic compound
- Inhibition of reuptake of neurotransmitters- serotonin and noradrenaline
- Modification of transmission of pain impulses
- Activation of descending serotonergic pathways
- Activation of noradrenergic pathways involved in analgesia
- Stimulation of mu-opioid receptors
- Modulation of central monoaminergic inhibitory pathways
Dosage & Administration
- Usual doses of 50 to 100 mg every four to six hours
- Initial dose of 100 mg for acute pain
- Initial dose of 50 mg for chronic painful conditions
- Total daily dosage by mouth should not exceed 400 mg
- Dosage for sustained release capsule or tablet
- Dose of 50-100 mg per 4 to 6 hours for injection
- Dose of 100 mg Tramadol Hydrochloride 6 hourly for suppository
Interaction
- Monoamine oxidase (MAO) inhibitors not recommended for combination
- Concomitant administration of carbamazepine causes significant increase in Zydol metabolism
Contraindications
- Hypersensitivity to the drug
- Acute intoxication with alcohol, hypnotics, centrally acting analgesics, opioids, or psychotropic drugs

Pregnancy & Lactation
- Safe use in pregnancy not established
- Crosses the placenta
- No adequate and well-controlled studies in pregnant women
- Use during pregnancy only if potential benefit justifies the risk to the fetus
- Not to be administered during breastfeeding
Precautions & Warnings
- Respiratory depression
- Cautious administration in patients at risk for respiratory depression
- Not recommended for patients dependent on opioids
- Cautious use with concomitant CNS depressants
- Cautious use in patients with increased intracranial pressure or head injury
- Cautious use in patients with acute abdominal conditions
Use in Special Populations
- Dosing in children from the age of 1 year
- Suppository (100 mg Zydol) not to be administered in children and adolescents below the age of 14 years
- Zydol 100 mg SR Capsules not studied in children
