Indications
- Management of hypertension
- Long-term management of angina pectoris
- Managing hemodynamically stable patients with acute myocardial infarction
Pharmacology
- Beta1 selective (cardioselective) beta-adrenergic receptor antagonist
- Poor penetration of cell membranes due to high hydrophilicity
Dosage & Administration
- Initial dose: 50 mg once daily for hypertension
- Initial dose: 50 mg once daily for angina pectoris
- Initial dose: 5 mg intravenous injection for acute myocardial infarction
Interactions
- Additive effect with catecholamine-depleting drugs
- Additive effect with calcium channel blockers
- Risk of severe bradycardia when combined with disopyramide
- Potential additive effects with amiodarone
- Potential exacerbation of rebound hypertension with clonidine withdrawal
- Decreased hypotensive effects with prostaglandin synthase inhibiting drugs
- Increased risk of severe allergic reaction in patients with history of anaphylaxis
Contraindications
- Sinus bradycardia
- Heart block greater than first degree
- Cardiogenic shock
- Overt cardiac failure
- Hypersensitivity to atenolol or its components
Side Effects
- Agranulocytosis
- Allergic reactions like fever, sore throat, respiratory distress
- Reversible mental depression and other CNS effects
- Mesenteric arterial thrombosis, ischemic colitis
- Skin rashes and dry eyes
- Erythematous rash
Pregnancy & Lactation
Pregnancy Category D, caution during nursing
Precautions & Warnings
- Evaluation in patients already on beta blocker
- Aggravation of peripheral arterial circulatory disorders
- Caution in patients with impaired renal function
Use in Special Populations
Dosage adjustment in elderly and renal impaired patients
Overdose Effects
Symptoms include lethargy, respiratory disorders, bradycardia
