Indications

  • Moderate to severe painful conditions
  • Postoperative pain
  • Colic and spastic pain
  • Cancer pain
  • Joint pain

  • Pain associated with osteoporosis

Pharmacology

  • Centrally acting synthetic analgesic compound
  • Inhibits reuptake of neurotransmitters
  • Modifies pain impulse transmission
  • Stimulation of mu-opioid receptors

Dosage & Administration

  • Usual doses of 50 to 100 mg every four to six hours
  • Initial dose of 100 mg for acute pain
  • Initial dose of 50 mg for chronic painful conditions
  • Maximum total daily dosage of 400 mg
  • Sustained Release Capsule - one every 12 hours
  • Injection dose of 50-100 mg every 4 to 6 hours
  • Suppository - 100 mg every 6 hours
  • Higher doses for cancer pain and severe post-operative pain

Interaction

  • MAO inhibitors not recommended for combination
  • Concomitant administration of carbamazepine increases metabolism

Contraindications

  • Hypersensitivity to the drug
  • Acute intoxication with alcohol, opioids, or psychotropic drugs

Precautions & Warnings

  • Respiratory depression risk
  • Not recommended for opioid dependent patients
  • Cautious use with CNS depressants
  • Cautious use with MAO inhibitors
  • Cautious use in patients with increased intracranial pressure or head injury
  • Cautious use in patients with acute abdominal conditions

Use in Special Populations

  • Dose for children from the age of 1 year - 1-2 mg/kg body weight
  • Suppository (100 mg Zydol) not for children and adolescents below the age of 14 years
