Indications
- Treatment of moderate to severe painful conditions
- Postoperative pain
- Colic and spastic pain
- Cancer pain
- Joint pain
- Neck and back pain
- Pain associated with osteoporosis
Pharmacology
- Centrally acting synthetic analgesic compound
- Inhibits reuptake of neurotransmitters (serotonin and noradrenaline)
- Modifies transmission of pain impulses
- Activates descending serotonergic pathways
- Involves mu-opioid receptors and central monoaminergic inhibitory pathways
Dosage & Administration
- Usual doses of 50 to 100 mg every four to six hours
- Initial dose of 100 mg for acute pain
- Initial dose of 50 mg for chronic painful conditions
- Sustained Release Capsule or Tablet for every 12 hours
- Injection of 50-100 mg every 4 to 6 hours
- Tramadol suppository for adults at 100 mg every 6 hours
- Maximum total daily dosage should not exceed 400 mg
Interaction
- Monoamine oxidase (MAO) inhibitors is not recommended for combination with Opidol
- Concomitant administration of carbamazepine with Opidol increases Opidol metabolism
Contraindications
- Hypersensitivity to Tramadol
- Acute intoxication with alcohol, hypnotics, centrally acting analgesics, opioids, or psychotropic drugs

Pregnancy & Lactation
- Safe use in pregnancy has not been established
- Tramadol crosses the placenta
- Tramadol and its metabolites detected in breast milk
Precautions & Warnings
- Cautious administration in patients at risk for respiratory depression
- Not recommended for patients dependent on opioids
- Caution when administering with CNS depressants
- Use with caution in patients taking MAO inhibitors
- Use with caution in patients with increased intracranial pressure, head injury, or acute abdominal conditions
Use in Special Populations
- Can be given in a dose of 1-2 mg/kg body weight in children from the age of 1 year
- Suppository (100 mg Opidol) not to be administered in children and adolescents below the age of 14 years
- Opidol 100 mg SR Capsules not studied in children
