Indications

  • Treatment of moderate to severe painful conditions
  • Postoperative pain
  • Colic and spastic pain
  • Cancer pain
  • Joint pain
  • Neck and back pain
  • Pain associated with osteoporosis

Pharmacology

  • Centrally acting synthetic analgesic compound
  • Inhibition of reuptake of neurotransmitters- serotonin and noradrenaline
  • Modification of transmission of pain impulses
  • Stimulation of mu-opioid receptors

Dosage & Administration

  • Usual doses of 50 to 100 mg every four to six hours
  • Initial dose of 100 mg for acute pain and 50 mg for chronic painful conditions
  • Total daily dosage by mouth not to exceed 400 mg
  • Sustained Release Capsule to be taken every 12 hours
  • Injection dose of 50-100 mg every 4 to 6 hours
  • Tramadol suppository for adults is 100 mg every 6 hourly

Interaction

  • Contraindicated with monoamine oxidase (MAO) inhibitors
  • Significant increase in metabolism when combined with carbamazepine

Contraindications

  • Hypersensitivity to Tramadol
  • Contraindicated in acute intoxication with certain drugs

Side Effects

  • Common: dizziness/vertigo, nausea, constipation, headache, somnolence, vomiting
  • Less common: malaise, weight loss, abdominal pain, GI bleeding

Pregnancy & Lactation

  • Safe use not established in pregnancy
  • Crosses placenta
  • Not recommended during breastfeeding

Precautions & Warnings

  • Administration cautious in patients at risk for respiratory depression
  • Not recommended for opioid-dependent patients
  • Should be used with caution in patients receiving CNS depressants or MAO inhibitors
  • Caution in patients with increased intracranial pressure

Use in Special Populations

  • Can be given in children from the age of 1 year
  • Suppository not to be administered in children and adolescents below the age of 14 years
