Indications

  • Management of moderate to moderately severe pain in adults
  • Short-term management of acute pain

Pharmacology

  • Paracetamol with analgesic and antipyretic properties
  • Weak anti-inflammatory activity
  • Thought to act primarily in the CNS
  • Increases pain threshold by inhibiting cyclooxygenase (COX-1, COX-2, COX-3) enzymes
  • Tramadol - centrally acting synthetic opioid analgesic
  • Mode of action not completely understood
  • Binding to μ-opioid receptors
  • Inhibition of norepinephrine and serotonin reuptake

Dosage & Administration

  • Recommended dose for moderate to moderately severe pain
  • Recommended dose for short-term (five days or less) management of acute pain
  • Can be administered without regard to food

Contraindications

  • Hypersensitivity to tramadol, paracetamol, or opioids
  • Contraindicated in situations where opioids are contraindicated

Precautions & Warnings

  • May impair mental or physical abilities
  • Should not be taken with alcohol
  • Should not be taken in combination with other tramadol or paracetamol-containing products
  • Use with caution when taking tranquilizers, hypnotics or other opiate containing analgesics

Use in Special Populations

  • Safety and effectiveness not studied in the pediatric population
  • Dose selection for elderly patients should be cautious
  • Not studied in patients with impaired renal function
  • In patients with creatinine clearances of less than 30 ml/min, dosing interval should be increased
  • Not studied in patients with impaired hepatic function
  • Not recommended for use in patients with hepatic impairment
