Pharmacology

  • Potent non-steroidal anti-inflammatory drug (NSAID)
  • Pronounced anti-rheumatic, anti-inflammatory, analgesic and antipyretic properties
  • Uricosuric effect
  • Inhibits prostaglandin biosynthesis causing inflammation, pain, and fever
  • Rapid and complete absorption from the gastrointestinal tract
  • Metabolized in the liver and undergoes first pass metabolism

Dosage and Administration

  • Diclofenac FC Tablet: Adults: 75-150 mg daily in 2 to 3 divided doses, preferably after food
  • Diclofenac SR Tablet: 1 tablet daily for adults, 1-3 mg of diclofenac/kg body wt. daily in divided doses for children, lowest effective dosage recommended for elderly patients
  • Diclofenac Dispersible Tablet: 2-3 tablets daily for adults, 1-3 mg/kg body weight for children, to be dropped into a half-glass of water and stirred to aid dispersion before swallowing
  • Diclofenac TR Capsule: One capsule daily, to be taken preferably after mealtimes
  • Diclofenac Suppository: 50 mg 2-3 times daily for adults, maximum daily dose is 150 mg
  • Diclofenac injection: 1 ampoule daily, increased to 2 ampoules daily in serious cases
  • Diclofenac Gel: 2-4 g applied to the skin 3-4 times daily, for external use only, rubbed in lightly, may also be given in addition to further treatment with other dosage forms of Diclofenac

Interaction

  • May have drug interactions with Lithium, digoxin, anticoagulants, antidiabetic agents, cyclosporin, methotrexate, quinolone antimicrobials, other NSAIDs and steroids

Contraindications

  • Hypersensitivity to any ingredient of the product
  • Peptic ulcer
  • Hypersensitivity to Diclofenac like other non-steroid anti-inflammatory agents
  • Contra-indicated in asthmatic patients where attack with asthma, urticaria, or acute rhinitis are precipitated by acetylsalicylic acid or by other drugs with prostaglandin synthetase inhibitor
  • Not to be used under occlusive airtight dressings

Pregnancy and Lactation

  • Should be employed only for compelling reasons during pregnancy
  • Lowest effective dose should be used
  • Not recommended during the first trimester of pregnancy
  • Insignificant quantity detected in breast milk with no undesirable effects on the infant

Precautions and Warnings

  • In rare instances of peptic ulceration or gastrointestinal bleeding, close observation in advanced age patients
  • Not to come in contact with eyes or mucus membranes, to be washed off the hands properly after application, not to be taken by mouth
