Indications
- Relief of pain and inflammation

Pharmacology
- Potent nonsteroidal antiinflammatory drug (NSAID)
- Marked analgesic and antipyretic properties
- Inhibits prostaglandin synthesis
- Peak plasma concentration achieved within half an hour
Dosage & Administration
- Adult dosage: one ampoule once or twice daily by intramuscular injection
- Renal colic dosage: one ampoule once daily intramuscularly, second dose after 30 minutes if necessary
- Children's dosage for juvenile chronic arthritis
- Dosage recommendation for elderly patients
Interaction
- Increases plasma concentrations of Lithium and Digoxin
- Isolated reports of increased risk of hemorrhage with anticoagulant therapy
- Can be given together with oral antidiabetic agents
- Possible nephrotoxicity when combined with Cyclosporin
- Serious toxicity reported with concomitant use of Methotrexate and NSAIDs
- Potential interaction with quinolone antimicrobials leading to convulsions
- Possible increase in unwanted effects with co-administration of other NSAIDs and steroids
- Inhibition of diuretics activity
- Possible increase in serum potassium levels with potassium-sparing diuretics
Contraindications
- Hypersensitivity to Diclofenac
- Active or suspected peptic ulcer or gastrointestinal bleeding
- Previous attacks of asthma, urticaria or acute rhinitis
- Hypersensitivity to local anaesthetics of the amide type
- Adams-Stokes syndrome or severe degrees of heart block
- Hypersensitivity to excipients used in the formulation
Side Effects
- Mild and transient side effects
- Gastrointestinal discomfort, nausea, diarrhea, bleeding
- Injection site disorder, abscesses, local necrosis
- CNS reactions such as drowsiness, dizziness, disorientation, confusion
Pregnancy & Lactation
- Not recommended unless compelling reasons exist
- Lowest effective dosage should be used
- Not recommended during the last trimester
- Small quantities may be detected in breast milk with no expected undesirable effects on the infant
Precautions & Warnings
- Close observation for patients with severe hepatic, cardiac or renal insufficiency or the elderly
- Monitoring of renal function, lowest effective dose usage
- Discontinuation in case of abnormal liver function tests, clinical signs or symptoms consistent with liver disease
- Precautionary monitoring for patients on long term NSAID treatment
