Indications
- Management of hypertension

Pharmacology
- Beta1 selective (cardioselective) beta-adrenergic receptor antagonist
- Markedly hydrophilic
- Poor penetration of cell membranes
Dosage & Administration
- Initial dose of 50 mg for hypertension
- Initial dose of 50 mg for angina pectoris
- Initial intravenous dose of 5 mg for acute myocardial infarction
Interaction
- Additive effect with catecholamine-depleting drugs
- Additive effect with calcium channel blockers
- Potential adverse effects with disopyramide
- Additive effect with amiodarone
- Exacerbation of rebound hypertension with clonidine withdrawal
- Decreased hypotensive effects with prostaglandin synthase inhibitors
- Increased risk of severe allergic reaction in patients with history of anaphylaxis
- Increased risk of bradycardia with concomitant use of digitalis glycosides
Contraindications
- Sinus bradycardia
- Heart block greater than first degree
- Cardiogenic shock
- Overt cardiac failure
- History of hypersensitivity to atenolol or its components

- Agranulocytosis
- Fever and respiratory distress
- Reversible mental depression
- Mesenteric arterial thrombosis
- Skin rashes and dry eyes
- Erythematous rash
Pregnancy & Lactation
- Pregnancy Category D
- Caution in nursing women
- Potential adverse effects in premature or impaired infants
Precautions & Warnings
- Caution in patients already on beta blockers
- Aggravation of peripheral arterial circulatory disorders
- Caution in patients with impaired renal function
- Dose selection for elderly patients
- Assessment of renal function in hypertension and myocardial infarction
Use in Special Populations
- Dosage adjustment in severe renal impairment
- Lower starting dose for elderly or renal impaired patients

Overdose Effects
- Reported overdoses up to 5g
- Symptoms include lethargy, bradycardia, hypotension, bronchospasm
- Treatment with induced emesis, gastric lavage, activated charcoal, hemodialysis
