Indications
- Relief of pain and inflammation in various conditions including arthritic conditions, musculoskeletal disorders, trauma-related pain, postoperative pain, renal colic, etc.
Pharmacology
- Potent nonsteroidal antiinflammatory drug (NSAID) with analgesic and antipyretic properties
- Inhibition of prostaglandin synthesis
- Uricosuric effects
- Action associated with inhibition of cyclooxygenase
- Peak plasma concentration achieved within half an hour following injection
- Lidocaine acts as a local anaesthetic and minimizes pain at the injection site
Dosage & Administration
- Adult dosage: one ampoule once daily by intramuscular injection, and in severe cases, twice daily
- Renal colic: one ampoule once daily intramuscularly, with an option for a second dose after 30 minutes if necessary
- Children dosage: 1-3 mg of Diclofenac Sodium per kg body weight daily in divided doses
- Recommended lowest effective dosage for elderly patients
Interaction
- Potential interactions with Lithium, Digoxin, Anticoagulants, Antidiabetic agents, Cyclosporin, Methotrexate, Quinolone antimicrobials, other NSAIDs and steroids, and diuretics
Contraindications
- Hypersensitivity to Diclofenac and local anaesthetics of the amide type
- Active or suspected peptic ulcer, gastrointestinal bleeding
- Attacks of asthma, urticaria, acute rhinitis precipitated by Aspirin or other NSAIDs
- Adams-Stokes syndrome, severe degrees of heart block, hypersensitivity to excipients used in the formulation
Side Effects
- Mild and transient gastrointestinal discomfort, nausea, diarrhea, and bleeding
- Rare instances of injection site disorder, abscesses, and local necrosis
- Adverse effects due to Lidocaine involving the CNS
Pregnancy & Lactation
- Should not be prescribed during pregnancy unless compelling reasons exist
- Lowest effective dosage should be used, not recommended during the last trimester of pregnancy
- Small quantities may be detected in breast milk with no expected undesirable effects on the infant
Precautions & Warnings
- Close observation for patients with severe hepatic, cardiac, or renal insufficiency, and the elderly
- Monitoring renal function, lowest effective dose usage, and precautionary measures for long-term treatment with NSAIDs
