Pharmacology

  • Potent nonsteroidal antiinflammatory drug (NSAID)

Dosage & Administration

  • Adult dosage: once or twice daily by intramuscular injection
  • Renal colic dosage
  • Children dosage for juvenile chronic arthritis
  • Lowest effective dosage recommended for elderly patients

Interaction

  • Increase in plasma concentrations of Lithium and Digoxin
  • Isolated reports of increased risk of haemorrhage with anticoagulant therapy
  • Compatibility with oral antidiabetic agents
  • Cases of nephrotoxicity reported with concomitant use of Cyclosporin
  • Potential serious toxicity when combined with Methotrexate
  • Possibility of convulsions with quinolone antimicrobials
  • Potential interaction with diuretics leading to increased serum potassium levels

Contraindications

  • Hypersensitivity to Diclofenac
  • Active or suspected peptic ulcer or gastrointestinal bleeding
  • Precipitation of asthma, urticaria or acute rhinitis by NSAIDs
  • Hypersensitivity to amide type local anaesthetics
  • Patients with specific heart conditions
  • Hypersensitivity to excipients used in the formulation

Side Effects

  • Bleeding
  • Injection site disorders
  • Abscesses and local necrosis in rare instances
  • Transient CNS effects like drowsiness, dizziness, confusion

Pregnancy & Lactation

  • Not recommended during pregnancy or last trimester
  • Lowest effective dosage to be used if necessary
  • Detection of Diclofenac in breast milk with no expected undesirable effects on the infant

Precautions & Warnings

  • Close observation of patients with severe hepatic, cardiac or renal insufficiency
  • Monitoring of renal function in patients using NSAIDs
  • Discontinuation of Diclofenac on abnormal liver function tests or other manifestations
  • Long-term monitoring of patients using NSAIDs
