Indications
- Relieves pain and inflammation in arthritic conditions
- Manages acute musculoskeletal disorders
- Treats painful conditions resulting from trauma
- Serves as local anesthetic for injection site
Pharmacology
- Potent nonsteroidal antiinflammatory drug
- Marked analgesic and antipyretic properties
- Inhibits prostaglandin synthesis
- Rapid peak plasma concentration
Dosage & Administration
- Adult dosage: Once daily by intramuscular injection
- Special dosage for renal colic
- Children dosage for juvenile chronic arthritis
- Recommended lowest effective dosage for elderly patients
Interaction
- May increase plasma concentrations of Lithium and Digoxin
- Isolated reports of increased risk of hemorrhage with anticoagulants
- Can be given together with oral antidiabetic agents without influencing their clinical effect
- Cases of nephrotoxicity reported when given concomitantly with Cyclosporin
- Serious toxicity reported when given within 24 hours of Methotrexate
- Caution required when considering concomitant therapy of quinolones and NSAIDs
- Co-administration with other NSAIDs and steroids may increase unwanted effects
- Inhibits the activity of diuretics
Contraindications
- Hypersensitivity to Diclofenac or local anesthetics
- Active or suspected peptic ulcer or gastrointestinal bleeding
- Precipitation of asthma, urticaria, or acute rhinitis by Aspirin or other NSAIDs
- Presence of Adams-Stokes syndrome or severe degrees of heart block
- Hypersensitivity to any of the excipients used in the formulation
Side Effects
- Mild and transient side effects
- Gastrointestinal discomfort, nausea, diarrhea, and occasional bleeding
- Rare instances of injection site disorder, abscesses, and local necrosis
- CNS-related adverse effects due to Lidocaine
Pregnancy & Lactation
- Not recommended during pregnancy unless compelling reasons
- Lowest effective dosage should be used
- Not recommended during the last trimester of pregnancy
- Very small quantities may be detected in breast milk with no expected undesirable effects on the infant
Precautions & Warnings
- Close observation for patients with severe hepatic, cardiac, or renal insufficiency or the elderly
- Lowest effective dose should be used for renal insufficiency
- Monitoring of renal, hepatic function, and blood counts for long-term NSAID treatment
