Pharmacology
- Potent non-steroidal anti-inflammatory drug (NSAID)
- Pronounced anti-rheumatic, anti-inflammatory, analgesic, and antipyretic properties
- Uricosuric effect
- Inhibits prostaglandin biosynthesis
- Rapid and complete absorption from gastro-intestinal tract
- Metabolized in the liver
- Undergoes first-pass metabolism
Dosage & Administration
- Adults: 75-150 mg daily in 2 to 3 divided doses, preferably after food
- Children: 1-3 mg of diclofenac/kg body wt. daily in divided doses
- Elderly patients: Lowest effective dosage recommended
- Maximum daily dose for Diclofenac Dispersible Tablet is 150 mg
- Diclofenac Gel: 2-4 g applied to the skin 3-4 times daily
- Diclofenac Suppository: 50 mg 2-3 times daily
- Diclofenac injection: 1-2 ampoules daily
- Diclofenac TR Capsule: One capsule daily
Interaction
- May increase plasma concentrations of lithium and digoxin
- Increased risk of hemorrhage with combined use of diclofenac and anticoagulant therapy
- Can be given together with oral antidiabetic agents without influencing their clinical effect
- Cases of nephrotoxicity reported in patients receiving cyclosporin and diclofenac concomitantly
- Serious toxicity reported when methotrexate and NSAIDs given within 24 hours of each other
- Convulsions may occur due to an interaction between quinolones and NSAIDs
- Co-administration with other systemic NSAIDs and steroids may increase frequency of unwanted effects
Contraindications
- Hypersensitivity to any ingredient of the product
- Peptic ulcer
- Hypersensitivity to Diclofenac or other non-steroid anti-inflammatory agents
- Contra-indicated in asthmatic patients where attack with asthma, urticaria, or acute rhinitis are precipitated by acetylsalicylic acid or other drugs with prostaglandin synthetase inhibitor
- Not to be used under occlusive airtight dressings

Pregnancy & Lactation
- Should be employed only for compelling reasons during pregnancy
- Lowest effective dose should be used
- Not recommended during the first trimester of pregnancy
- Diclofenac Sodium Gel is not recommended during pregnancy
- Very insignificant quantity may be detected in breast milk
Precautions & Warnings
- Close observation in rare instances of peptic ulceration or gastrointestinal bleeding
- Advanced age patients should be kept under close observation
- Not to be allowed to come in contact with eyes or mucous membranes
- Hands should be washed properly after application
- Not to be taken by mouth
