Pharmacology
Potent non-steroidal anti-inflammatory drug (NSAID) with pronounced anti-rheumatic, anti-inflammatory, analgesic and antipyretic properties, and some uricosuric effect. Inhibits prostaglandin biosynthesis which causes inflammation, pain, and fever. Rapid and complete absorption from the gastrointestinal tract. Metabolized in the liver and undergoes first-pass metabolism.
Dosage & Administration
- Adults: 75-150 mg daily in 2 to 3 divided doses
- Children: 1-3 mg of diclofenac/kg body wt. daily in divided doses
- Elderly patients: Lowest effective dosage recommended
- Diclofenac Gel: 2-4 g applied to the skin 3-4 times daily
- Diclofenac TR Capsule: One capsule daily
- Diclofenac Suppository: 50 mg suppository 2-3 times daily
- Diclofenac injection: 1-2 ampoules daily
Interaction
- Increased plasma concentrations of lithium and digoxin
- Increased risk of haemorrhage with anticoagulant therapy
- Can be given together with oral antidiabetic agents without influencing their clinical effect
- Cases of nephrotoxicity with cyclosporin
- Serious toxicity when methotrexate and NSAIDs are given within 24 hours of each other
- Convulsions may occur due to an interaction between quinolones and NSAIDs
- Co-administration with other systemic NSAIDs and steroids may increase frequency of unwanted effects
Contraindications
- Hypersensitivity to any ingredient
- Peptic ulcer
- Hypersensitivity to Diclofenac like other non-steroid anti-inflammatory agents
- Asthmatic patient in whom attack with asthma, urticaria, or acute rhinitis are precipitated by acetylsalicylic acid or other drugs with prostaglandin synthetase inhibitor
- Not to be used under occlusive airtight dressings
Side Effects
- Mild, rare and transient adverse effects
- Epigastric pain, eructation, nausea, diarrhea, dizziness, headache, peripheral edema, skin reactions, rash, eczema, local irritation and reddening of the skin, skin rash
Pregnancy & Lactation
- Use only for compelling reasons during pregnancy
- Lowest effective dose should be used
- Not recommended during the first trimester of pregnancy
- Not recommended during lactation due to insignificant quantity detected in breast milk
Precautions & Warnings
- Close observation in rare instances of peptic ulceration or gastrointestinal bleeding
- Elderly patients should be kept under close observation
- Not to come in contact with the eyes or mucus membranes
