Pharmacology
- Potent non-steroidal anti-inflammatory drug (NSAID)
- Pronounced anti-rheumatic, anti-inflammatory, analgesic and antipyretic properties
- Uricosuric effect
- Inhibits prostaglandin biosynthesis
- Rapid and complete absorption from the gastro-intestinal tract
- Metabolized in the liver
- First pass metabolism
Dosage & Administration
- Adult dosage: 75-150 mg daily in 2 to 3 divided doses
- Dose should be reduced in long term use
- Maximum daily dose: 150 mg
- Diclofenac SR Tablet: 1 tablet daily
- Children dosage: 1-3 mg of diclofenac/kg body weight daily
- Reduced dosage for elderly patients
- Diclofenac Dispersible Tablet: 2-3 tablets daily
- Maximum daily dose: 150 mg
- Diclofenac TR Capsule: One capsule daily
- Diclofenac Suppository dosage: 50 mg 2-3 times daily
- Maximum daily dose: 150 mg
- Diclofenac injection dosage: 1 ampoule daily, may be increased to 2 ampoules daily in serious cases
- External use only for Diclofenac Gel
- 2-4 g of Diclofenac gel should be applied to the skin 3-4 times daily
Interaction
- May interact with lithium and digoxin, increasing plasma concentrations
- Isolated reports of increased risk of haemorrhage with anticoagulants
- No influence on antidiabetic agents' clinical effect
- Cases of nephrotoxicity reported with cyclosporin concomitant use
- Serious toxicity reported with methotrexate and NSAIDs given within 24 hours
- Caution required with quinolone antimicrobials
- Increased frequency of unwanted effects with co-administration of other systemic NSAIDs and steroids
- Lower plasma levels when co-administered with aspirin
Contraindications
- Hypersensitivity to any ingredient of the product
- Peptic ulcer
- Hypersensitivity to Diclofenac or other non-steroid anti-inflammatory agents
- Contra-indicated in asthmatic patients with precipitated asthma attack, urticaria or acute rhinitis
Side Effects
- Mild, rare and transient adverse effects

- Eructation
- Nausea
- Diarrhea
- Dizziness
- Headache
- Peripheral edema
- Skin reactions such as rash and eczema
- Local irritation
- Reddening of the skin
- Skin rash
Pregnancy & Lactation
- Should be employed for compelling reasons only during pregnancy
- Lowest effective dose should be used
- Not recommended during the first trimester of pregnancy
- Not recommended during pregnancy due to insufficient clinical data
- Insignificant quantity may be detected in breast milk
Precautions & Warnings
- Close observation required in rare instances of peptic ulceration or gastrointestinal bleeding
- Close observation in patients with advanced age
- Avoid contact with eyes or mucous membranes for Renac TR Gel
- Hands should be washed properly after application
- Not to be taken by mouth
