Pharmacology

Dosage & Administration

  • Adults: 75-150 mg daily in 2 to 3 divided doses, preferably after food
  • Dose should be reduced in long term use
  • Children: 1-3 mg of diclofenac/kg body wt. daily in divided doses
  • In elderly or debilitated patients, the lowest effective dosage is recommended
  • 2-3 tablets for adults, maximum daily dose is 150 mg
  • 1 capsule daily, preferably after mealtimes
  • For adults: 50 mg suppository 2-3 times daily, maximum daily dose is 150 mg
  • For external use only: 2-4 g of Diclofenac gel should be applied to the skin 3-4 times daily

Interactions

  • Lithium and digoxin may increase plasma concentrations
  • Anticoagulants may increase risk of haemorrhage
  • Can be given together with oral antidiabetic agents without influencing their clinical effect
  • Nephrotoxicity reported in patients receiving cyclosporin and diclofenac concomitantly
  • Serious toxicity reported when methotrexate and NSAIDs are given within 24 hours of each other
  • Convulsions may occur due to an interaction between quinolones and NSAIDs
  • Co-administration of diclofenac with other systemic NSAIDs and steroids may increase the frequency of unwanted effects

Contraindications

  • Hypersensitive to any ingredient of the products
  • Peptic ulcer
  • Hypersensitivity to Diclofenac or other non-steroid anti-inflammatory agents
  • Contra-indicated in asthmatic patient in whom attack with asthma, urticaria or acute rhinitis are precipitated by acetylsalicylic acid or by other drugs with prostaglandin synthetase inhibitor
  • Gel should not be used under occlusive airtight dressings

Pregnancy & Lactation

  • Should be employed only for compelling reasons during pregnancy
  • The lowest effective dose should be used
  • Not recommended during the first trimester of pregnancy
  • Not recommended during pregnancy as insufficient clinical data exists
  • A very insignificant quantity may be detected in breast milk with no undesirable effects on the infant

Precautions & Warnings

  • Rare instances of peptic ulceration or gastrointestinal bleeding
  • Close observation for patients with advanced age
  • Gel should not contact eyes or mucus membranes
  • Hands should be washed properly after application
  • Not to be taken by mouth
