Pharmacology

  • Non-steroidal anti-inflammatory drug (NSAID)

  • Rapid and complete absorption from the gastro-intestinal tract

Dosage & Administration

  • Adult dosage: 75-150 mg daily
  • Dosage reduction in long term use
  • Children dosage: 1-3 mg of diclofenac/kg body wt. daily
  • Elderly patients dosage: Lowest effective dosage
  • Maximum adult daily dose: 150 mg
  • Administration: Preferably after meals
  • Suppository dosage: 50 mg 2-3 times daily
  • Gel application: 2-4 g 3-4 times daily

Interaction

  • May increase plasma concentrations of lithium and digoxin
  • Reports of increased risk of haemorrhage with combined use of diclofenac and anticoagulant therapy
  • No influence on clinical effect of oral antidiabetic agents
  • Caution with concomitant therapy of NSAID and quinolones
  • Increase in unwanted effects with co-administration of diclofenac with other systemic NSAIDs and steroids
  • Lowered plasma levels when co-administered with aspirin

Contraindications

  • Hypersensitivity to any ingredient
  • Peptic ulcer
  • Asthmatic patients with attacks precipitated by acetylsalicylic acid or other drugs with prostaglandin synthetase inhibitor
  • Avoid use under occlusive airtight dressings

  • Mild, rare and transient adverse effects
  • Epigastric pain
  • Eructation
  • Nausea
  • Diarrhea
  • Dizziness
  • Headache
  • Peripheral edema
  • Skin reactions such as rash and eczema
  • Local irritation and reddening of the skin with gel application

Pregnancy & Lactation

  • Use during pregnancy for compelling reasons only
  • Lowest effective dose should be used
  • Avoid during the first trimester
  • Not recommended during pregnancy or lactation

Precautions & Warnings

  • Close observation for peptic ulceration or gastrointestinal bleeding
  • Close observation for advanced age patients
  • Avoid contact with eyes or mucus membranes for gel application
  • Hands should be washed properly after application
  • Avoid oral ingestion
