Pharmacology
- Potent non-steroidal anti-inflammatory drug (NSAID) with pronounced anti-rheumatic
- Anti-inflammatory
- Analgesic
- Antipyretic properties
- Has some uricosuric effect
- Inhibits prostaglandin biosynthesis
- Rapid and complete absorption from the gastro-intestinal tract
- Metabolized in the liver and undergoes first-pass metabolism
Dosage & Administration
- Adult dosage: 75-150 mg daily in 2 to 3 divided doses, preferably after food
- Dose reduction in long term use
- Children dosage: 1-3 mg of diclofenac/kg body weight daily in divided doses
- Maximum daily dose of diclofenac is 150 mg
- Dosage varies for different formulations like FC Tablet, SR Tablet, Dispersible Tablet, TR Capsule, Suppository, Injection, Gel
Interaction
- Increased plasma concentrations of lithium and digoxin
- Isolated reports of an increased risk of hemorrhage when combined with anticoagulant therapy
- Can be given together with oral antidiabetic agents without influencing their clinical effect
- Nephrotoxicity reported in patients receiving cyclosporine and diclofenac concomitantly
- Serious toxicity reported with methotrexate and NSAIDs given within 24 hours of each other
- Caution needed with concomitant therapy of NSAID and quinolones
- Co-administration with other systemic NSAIDs and steroids may increase the frequency of unwanted effects
Contraindications
- Hypersensitivity to any ingredient of the product
- Peptic ulcer
- Hypersensitivity to Diclofenac or other non-steroid anti-inflammatory agents
- Contra-indicated in asthmatic patients where attacks are precipitated by acetylsalicylic acid or other drugs with prostaglandin synthetase inhibitor
- Not to be used under occlusive airtight dressings

- Peripheral edema
- Skin reactions such as rash and eczema
- Local irritation
- Reddening of the skin
Pregnancy & Lactation
- Should be used only for compelling reasons during pregnancy
- Lowest effective dose should be used
- Not recommended during the first trimester of pregnancy
- Not recommended during lactation due to insignificant quantity detected in breast milk
Precautions & Warnings
- Close observation in rare instances of peptic ulceration or gastrointestinal bleeding
- Close observation in patients of advanced age
- Avoid contact with eyes or mucous membranes
- Hands should be washed properly after application
- Not to be taken by mouth
