Description
Atorvastatin is indicated as an adjunct to diet to reduce elevated total cholesterol, LDL cholesterol, apolipoprotein B (Apo-B) and triglycerides levels in various diseases such as familial hypercholesterolaemia, mixed dyslipidemia, hypertriglyceridemia, dysbetalipoproteinaemia, and coronary artery disease.
Dosage and administration
- Adults: Usually 10 mg once daily; maximum 80 mg once daily
- Children (10-18 years): Initially 10 mg once daily, increased if necessary to max. 20 mg once daily
Interaction
- Coadministration of cyclosporine, fibric acid derivatives, niacin, erythromycin, azole antifungals may increase myopathy risk
- Antacid and colestipol may affect plasma concentrations of atorvastatin
- Digoxin levels may increase when coadministered with atorvastatin
- Atorvastatin may affect oral contraceptive AUC values, should be considered when selecting an oral contraceptive
Contraindications
- Hypersensitivity to any component of this medication
- Active liver disease or unexplained persistent elevations of serum transaminases
- History of serious adverse reaction to prior administration of HMG-CoA reductase inhibitors

Pregnancy and lactation
- Pregnancy: Atorvastatin is contraindicated during pregnancy due to potential congenital anomalies and toxicity to reproduction

Precautions and warnings
- Liver function tests should be performed before the initiation of treatment and periodically thereafter
- ATV should be used with caution in patients who consume substantial quantities of alcohol or have a history of liver disease
Mode of action
Selective inhibitor of HMG-CoA reductase, lowers plasma cholesterol and lipoprotein levels by inhibiting HMG-CoA reductase and cholesterol synthesis in the liver and increases the number of hepatic LDL receptors
Indications
- Reduction of total cholesterol and LDL cholesterol in patients with familial hypercholesterolaemia
- Treatment of patients with dysbetalipoproteinaemia and hypertriglyceridemia
- Reduction of cardiac ischaemic events in patients with asymptomatic or mild to moderate symptomatic coronary artery disease
Storage conditions
Keep in a dry place away from light and heat. Keep out of the reach of children.
Pharmacology
- Absorption: Rapidly absorbed after oral administration; maximum plasma concentrations occur within 1 to 2 hours
- Distribution: Mean volume of distribution is approximately 381 liters; 98% bound to plasma proteins
- Metabolism: Extensively metabolized to ortho- and parahydroxylated derivatives and various beta-oxidation products. Atorvastatin metabolism by cytochrome P450 3A4
- Excretion: Eliminated primarily in bile following hepatic and/or extra-hepatic metabolism
Use in special populations
- Geriatric: Plasma concentrations are higher in healthy elderly subjects compared to young adults
- Pediatric: Pharmacokinetic data in the pediatric population are not available
- Gender: No clinically significant difference in LDL-C reduction between men and women
- Renal insufficiency: No influence on the plasma concentrations or LDL-C reduction of ATV
- Hepatic insufficiency: In patients with chronic alcoholic liver disease, plasma concentrations of ATV are markedly increased
Overdose effects
Specific treatment is not available for atorvastatin overdose; patient should be treated symptomatically and supportive measures instituted
