Indications
- Short-term treatment for post-traumatic pain
- Management of post-operative pain and inflammation
- Treatment of painful and/or inflammatory conditions in gynaecology
- Relief from migraine attacks
- Management of painful syndromes of the vertebral column
- Use in non-articular rheumatism
- Adjuvant therapy in painful inflammatory infections of the ear, nose, or throat
Pharmacology
- Inhibition of cyclooxygenase (COX-1 and COX-2)
- Potent inhibitor of prostaglandin synthesis
- Sensitizes afferent nerves and potentiate the action of bradykinin
- Mediator of inflammation
Dosage & Administration
- Initial loading dose of 50 mg for adults
- 25-50 mg to be taken every eight hours if necessary for adults
- Initial loading dose of 50 mg for migraine, then further 25-50 mg after 2 hours if necessary
- Maximum daily dose of 150 mg for migraine
- Upto 75 mg daily in divided doses for children over 14 years of age
Interaction
- Potential drug interactions with Lithium and Digoxin
- Increased risk of hemorrhage with anticoagulant therapy
- Reports of nephrotoxicity with concomitant use of Cyclosporin
- Potential for serious toxicity when used with Methotrexate within 24 hours
- Inhibition of diuretic activity when used with diuretics
- Increased frequency of unwanted effects when co-administered with other NSAIDs and steroids
Contraindications
- Contraindicated in patients with known hypersensitivity to Diclofenac
- Not to be given to patients who have experienced asthma, urticaria, or allergic reactions after taking aspirin or other NSAIDs
- Contraindicated for perioperative pain in the setting of coronary artery bypass graft (CABG) surgery
Side Effects
- Mild and transient side-effects
- Occasional: epigastric pain, nausea, vomiting, diarrhea
- Rare: gastrointestinal bleeding, peptic ulcer, bloody diarrhea
Pregnancy & Lactation
- Not prescribed during pregnancy unless compelling reasons
- Lowest effective dosage should be used during pregnancy
- Not recommended during the last trimester of pregnancy
- Small quantities may be detected in breast milk
