Indications
- Short-term treatment for post-traumatic pain and inflammation
- Treatment for post-operative pain and swelling
- Management of gynaecological pain and inflammation
- Relief of migraine attacks
- Management of painful syndromes of the vertebral column
- Treatment for non-articular rheumatism
- Adjuvant treatment for painful inflammatory infections of the ear, nose, or throat
Pharmacology
- Inhibition of cyclooxygenase (COX-1 and COX-2)
- Potent inhibitor of prostaglandin synthesis in vitro
- Sensitization of afferent nerves by prostaglandins
- Mediation of inflammation by prostaglandins
Dosage & Administration
- Initial loading dose of 50 mg for adults
- 25-50 mg every eight hours for adults
- Additional dose of 25-50 mg for migraine after 2 hours
- Maximum daily dose of 150 mg for migraine
- Up to 75 mg daily in divided doses for children over 14 years
Interaction
- Increased plasma concentrations of Lithium and Digoxin
- Increased risk of haemorrhage with anticoagulant therapy
- Reported cases of nephrotoxicity with Cyclosporin concomitantly
- Serious toxicity when co-administered with Methotrexate
- Inhibition of diuretic activity
- Increased frequency of unwanted effects when co-administered with other NSAIDs and steroids
Contraindications
- Known hypersensitivity to Diclofenac
- History of asthma, urticaria, or allergic type reactions after taking aspirin or other NSAIDs
- Severe, rarely fatal, anaphylactic-like reactions to NSAIDs
- Treatment of perioperative pain in the setting of coronary artery bypass graft (CABG) surgery
Side Effects
- Mild and transient side effects
- Occasional: epigastric pain, gastrointestinal disorders, nausea, vomiting, diarrhea, abdominal cramps, dyspepsia, flatulence, anorexia
- Rare: gastrointestinal bleeding, peptic ulcer, bloody diarrhea
Pregnancy & Lactation
- Not recommended during pregnancy unless compelling reasons exist
- Lowest effective dosage should be used during pregnancy
- Not recommended during the last trimester of pregnancy
- Small quantities may be detected in breast milk with no expected undesirable effects on the infant
