Algirex 120 mg (Tablet)

Indications

• Symptomatic relief of Osteoarthritis (OA)
• Symptomatic relief of Rheumatoid arthritis (RA)
• Symptomatic relief of Ankylosing spondylitis
• Symptomatic relief of acute gouty arthritis
• Short-term treatment of moderate pain associated with dental surgery

Pharmacology

• Etoricoxib is a potent COX-2 specific inhibitor
• Inhibition of COX-2 decreases pain, inflammation, and fever
• Selective inhibition of COX-2 decreases potential for GI toxicity and effects on platelet aggregation

Dosage & Administration

• Recommended doses for different conditions
• Adjustment of dose for insufficient relief from symptoms
• Shortest duration possible and lowest effective daily dose should be used
• Periodic re-evaluation of patient's need for symptomatic relief and response to therapy

Interaction

• Interaction with oral anticoagulants, diuretics, ACE inhibitors, and Angiotensin II Antagonists
• Can be used concomitantly with acetylsalicylic acid at low doses
• Potential increase in nephrotoxic effects of ciclosporin or tacrolimus when coadministered with any NSAID
• Decreases lithium renal excretion and increases lithium plasma levels

Contraindications

• Hypersensitivity to the active substance or excipients
• Active peptic ulceration or active GI bleeding
• Pregnancy, lactation, severe hepatic dysfunction, estimated renal creatinine clearance <30 ml/min
• Children and adolescents under 16 years of age, inflammatory bowel disease, congestive heart failure
• Hypertension, established ischaemic heart disease, peripheral arterial disease, and/or cerebrovascular disease

Side Effects

• Palpitation, fatigue, influenza-like symptoms, ecchymosis
• Dry mouth, taste disturbance, mouth ulcer, appetite and weight change
• Atrial fibrillation, transient ischaemic attack, chest pain, flushing, cough, dyspnoea
• Anxiety, mental acuity impaired, paraesthesia, electrolyte disturbance, myalgia, arthralgia
• Confusion, hallucinations (very rare)

Pregnancy & Lactation

• Not recommended in women attempting to conceive
• Not known whether excreted in human milk
• Excreted in the milk of lactating rats, women must not breastfeed

Precautions & Warnings

• Caution with patients at risk of gastrointestinal complication with NSAIDs
• Careful consideration for patients with significant risk factors for cardiovascular events
• Monitoring of renal function, history of cardiac failure, left ventricular dysfunction, or hypertension
• Monitoring of liver function, discontinuation at first appearance of hypersensitivity
• May mask fever and signs of inflammation, caution when co-administering with warfarin or other oral anticoagulants

Overdose Effects

• No significant toxicity observed with single doses up to 500 mg and multiple doses up to 150 mg/day
• Usual supportive measures recommended in the event of overdose
