Indications
- Relief of Osteoarthritis
- Relief of Rheumatoid arthritis

Pharmacology
- Potent, orally active COX-2 specific inhibitor
- Inhibition of COX-2 decreases pain, inflammation, and fever symptoms
- Decreased potential for gastrointestinal toxicity and effects on platelet aggregation
- Dose-dependent inhibition of COX-2 without inhibition of COX-1
Dosage & Administration
- Recommended dose for Osteoarthritis: 30 mg once daily, increased to 60 mg if needed
- Recommended dose for Rheumatoid arthritis: 90 mg once daily
- Recommended dose for Ankylosing spondylitis: 90 mg once daily
- Recommended dose for acute gouty arthritis: 120 mg once daily for 8 days
- Recommended dose for postoperative dental surgery pain: 90 mg once daily for a maximum of 3 days
Interaction
- Increased prothrombin time with oral anticoagulants
- Potential reduction of diuretics and antihypertensive drugs effects with NSAIDs
- Concomitant use with low-dose acetylsalicylic acid for cardiovascular prophylaxis
- Potential increase in nephrotoxic effects with ciclosporin or tacrolimus coadministration
- Decreased lithium renal excretion leading to increased plasma levels
Contraindications
- Hypersensitivity to active substance or excipients
- Active peptic ulceration or gastrointestinal bleeding
- History of bronchospasm, acute rhinitis, nasal polyps, or allergic-type reactions to NSAIDs including COX-2 inhibitors
- Pregnancy and lactation
- Severe hepatic dysfunction
- Estimated renal creatinine clearance <30 ml/min
- Inflammatory bowel disease
- Congestive heart failure
- Uncontrolled hypertension
- Established ischaemic heart disease, peripheral arterial disease, or cerebrovascular disease

- Caution in patients at risk of gastrointestinal complications with NSAIDs
- Careful consideration for patients with significant risk factors for cardiovascular events
- Monitoring of renal function in patients at risk
- Caution in patients with cardiac failure, left ventricular dysfunction, or hypertension
- Monitoring of liver function and discontinuation if hepatic insufficiency occurs
- Discontinuation at the first sign of hypersensitivity
- Potential masking of fever and inflammation symptoms
- Caution when co-administering with warfarin or other oral anticoagulants
Overdose Effects
- Single doses up to 500 mg did not result in significant toxicity
- Usual supportive measures in case of overdose
