Indications
- Symptomatic relief of Osteoarthritis (OA)
- Symptomatic relief of Rheumatoid arthritis (RA)
- Symptomatic relief of Ankylosing spondylitis
- Symptomatic relief of acute gouty arthritis
- Short-term treatment of moderate pain associated with dental surgery
Pharmacology
- Potent, orally active cyclooxygenase-2 (COX-2) specific inhibitor
- Decreases clinical signs and symptoms with decreased potential for gastrointestinal toxicity and effects on platelet aggregation
Dosage & Administration
- Recommended dosage for Osteoarthritis: 30 mg once daily
- Recommended dosage for Rheumatoid arthritis: 90 mg once daily
- Recommended dosage for Ankylosing spondylitis: 90 mg once daily
- Recommended dosage for Acute gouty arthritis: 120 mg once daily
- Recommended dosage for postoperative dental surgery pain: 90 mg once daily, limited to a maximum of 3 days
Interaction
- May interact with oral anticoagulants
- May reduce the effect of diuretics and other antihypertensive drugs
- Can be used concomitantly with low-dose acetylsalicylic acid
- May increase the nephrotoxic effect of ciclosporin or tacrolimus
- Decreases lithium renal excretion and therefore increase lithium plasma levels
Contraindications
- Hypersensitivity to the active substance or to any of the excipients
- Active peptic ulceration or active gastrointestinal (GI) bleeding
- Patients who have experienced allergic-type reactions after taking acetylsalicylic acid or NSAIDs including COX-2 inhibitors
- Pregnancy and lactation
- Severe hepatic dysfunction
- Estimated renal creatinine clearance <30 ml/min
- Inflammatory bowel disease
- Congestive heart failure
- Patients with hypertension whose blood pressure is persistently elevated above 140/90 mmHg
- Established ischaemic heart disease, peripheral arterial disease, and/or cerebrovascular disease

Pregnancy & Lactation
- Not recommended in women attempting to conceive
- Not known whether excreted in human milk
- Not recommended for breastfeeding women
Precautions & Warnings
- Caution advised with treatment of patients at risk of developing gastrointestinal complications with NSAIDs
- Caution in patients with significant risk factors for cardiovascular events
- May cause reduction in prostaglandin formation and impaired renal function
- Caution in patients with a history of cardiac failure, left ventricular dysfunction, or hypertension
- Monitoring of renal function should be considered
- Monitoring advised for patients with symptoms and/or signs suggesting liver dysfunction
- Discontinue at the first appearance of skin rash or any other sign of hypersensitivity
- May mask fever and other signs of inflammation
Overdose Effects
- Single doses up to 500 mg and multiple doses up to 150 mg/day did not result in significant toxicity
- Usual supportive measures should be employed in the event of overdose
