Description
- Coxfree is indicated for the symptomatic relief of osteoarthritis, rheumatoid arthritis, ankylosing spondylitis, and acute gouty arthritis, as well as for the short-term treatment of moderate pain associated with dental surgery. It belongs to the therapeutic class of non-steroidal anti-inflammatory drugs (NSAIDs). The active substance is etoricoxib, a potent orally active cyclooxygenase-2 (COX-2) specific inhibitor.
Dose and administration
- Recommended doses for osteoarthritis, rheumatoid arthritis, ankylosing spondylitis, acute gouty arthritis, and postoperative dental surgery pain
- The shortest duration possible and the lowest effective daily dose should be used
- Re-evaluate the need for symptomatic relief and response to therapy periodically, especially in patients with osteoarthritis
Pharmacology
- Etoricoxib is a potent COX-2 specific inhibitor within the clinical dose range, selectively inhibits COX-2, decreasing clinical signs and symptoms with decreased potential for GI toxicity and effects on platelet aggregation.
Interaction
- Interaction with oral anticoagulants, diuretics, ACE inhibitors, angiotensin II antagonists, acetylsalicylic acid, ciclosporin, tacrolimus, and lithium
- Take without regards to meals
Contraindications
- Hypersensitivity to the active substance or excipients
- Active peptic ulceration or active gastrointestinal (GI) bleeding
- Patients with specific allergic-type reactions after taking acetylsalicylic acid or NSAIDs including COX-2 inhibitors
- Pregnancy and lactation
- Severe hepatic dysfunction and estimated renal creatinine clearance <30 ml/min
- Children and adolescents under 16 years of age
- Various medical conditions including inflammatory bowel disease, congestive heart failure, hypertension, established cardiovascular diseases
Side effects
- List of potential side-effects including palpitation, fatigue, influenza-like symptoms, ecchymosis, dry mouth, taste disturbance, mental acuity impaired, myalgia, arthralgia, confusion, hallucinations
Pregnancy lactation
- Not recommended in women attempting to conceive or during lactation due to unknown excretion in human milk and excretion in lactating rats
Precautions warnings
- Caution in patients at risk of GI complications with NSAIDs, elderly patients, and those with prior history of gastrointestinal disease
- Caution in patients with significant risk factors for cardiovascular events
- Monitoring of renal function and caution in patients with cardiac failure, left ventricular dysfunction, hypertension, or pre-existing edema
- Monitoring of liver function and discontinuation if signs of hepatic insufficiency occur
- Discontinuation at the first appearance of specific symptoms and/ or signs
- Potential for masking fever and other signs of inflammation when co-administered with warfarin or other oral anticoagulants
