Indications

  • Symptomatic relief of Osteoarthritis (OA)
  • Symptomatic relief of Rheumatoid arthritis (RA)
  • Symptomatic relief of Ankylosing spondylitis
  • Symptomatic relief of acute gouty arthritis pain and signs of inflammation
  • Short-term treatment of moderate pain associated with dental surgery

Pharmacology

  • Potent, orally active COX-2 specific inhibitor
  • Inhibition of COX-2 decreases pain, inflammation, and fever
  • Selective COX-2 inhibition decreases potential for gastrointestinal (GI) toxicity and platelet aggregation

Dosage & Administration

  • Recommended doses for different conditions (e.g., 30 mg for Osteoarthritis, 90 mg for Rheumatoid arthritis)
  • Varied duration and maximum doses for different conditions (e.g., 120 mg once daily for acute gouty arthritis, limited to a maximum of 3 days)
  • Caution about cardiovascular risks and need for re-evaluation of symptomatic relief

Interaction

  • Interaction with oral anticoagulants, diuretics, ACE inhibitors, Angiotensin II Antagonists, Acetylsalicylic Acid, Ciclosporin and tacrolimus, and Lithium

Contraindications

  • Hypersensitivity to the active substance or excipients
  • Active peptic ulceration or active gastrointestinal (GI) bleeding
  • Various allergic-type reactions
  • Pregnancy and lactation
  • Severe hepatic dysfunction, estimated renal creatinine clearance, children and adolescents under 16 years of age, inflammatory bowel disease, congestive heart failure, hypertension, established ischaemic heart disease, peripheral arterial disease, and/or cerebrovascular disease

Side Effects

  • Palpitation, fatigue, influenza-like symptoms, ecchymosis
  • Less commonly dry mouth, taste disturbance, mouth ulcer, appetite and weight change
  • Various less common and very rare side effects

Pregnancy & Lactation

  • Not recommended in women attempting to conceive
  • Not known whether excreted in human milk

Precautions & Warnings

  • Caution advised with treatment of patients at risk of developing GI complication with NSAIDs
  • Caution for patients with significant risk factors for cardiovascular events
  • Monitoring of renal function in patients with risk of impaired renal function
  • Caution for patients with history of cardiac failure, left ventricular dysfunction, hypertension, and pre-existing edema
  • Monitoring and discontinuation in case of hepatic insufficiency, skin rash, mucosal lesions, or other signs of hypersensitivity
  • Masking of fever and other signs of inflammation
  • Caution when co-administering with warfarin or other oral anticoagulants

Overdose Effects

  • Usual supportive measures in case of overdose
  • No significant toxicity observed with specified doses
