Description
Ecox is a selective COX-2 inhibitor indicated for the symptomatic relief of osteoarthritis, rheumatoid arthritis, ankylosing spondylitis, acute gouty arthritis, and short-term treatment of moderate pain associated with dental surgery. It is a non-steroidal anti-inflammatory drug (NSAID) that inhibits COX-2 without inhibiting COX-1 at doses up to 150 mg daily, with decreased potential for gastrointestinal toxicity and effects on platelet aggregation. It is available in 90 mg tablets and should be used with caution in patients with cardiovascular risks, renal impairment, hepatic dysfunction, and gastrointestinal complications.

Dosage
- Adult:
- Osteoarthritis: 30 mg once daily, increased to 60 mg for insufficient relief
- Rheumatoid arthritis: 90 mg once daily
- Ankylosing spondylitis: 90 mg once daily
- Acute gouty arthritis: 120 mg once daily for 8 days
- Postoperative dental surgery pain: 90 mg once daily, limited to a maximum of 3 days
- Adolescent over 16 years: Dosage similar to adults

Contraindications
- Hypersensitivity to the active substance or to any of the excipients
- Active peptic ulceration or active gastrointestinal (GI) bleeding
- Pregnancy and lactation
- Severe hepatic dysfunction
- Estimated renal creatinine clearance <30 ml/min
- Children and adolescents under 16 years of age
- Inflammatory bowel disease
- Congestive heart failure
- Patients with hypertension
- Established ischaemic heart disease, peripheral arterial disease, and/or cerebrovascular disease

Pregnancy & Lactation

Precautions & Warnings
- Caution in patients at risk of gastrointestinal complication with NSAIDs
- Caution in patients with significant risk factors for cardiovascular events
- Monitoring of renal function in patients with reduced renal blood flow
- Caution in patients with a history of cardiac failure, left ventricular dysfunction, or hypertension
- Monitoring of liver function and discontinuation in case of hepatic insufficiency
- Discontinuation at the first appearance of hypersensitivity signs
- Caution when co-administering with warfarin or other oral anticoagulants
Overdose Effects
No significant toxicity observed up to 150 mg/day for 21 days; employ usual supportive measures in case of overdose
