Indications
- Osteoarthritis (OA)
- Rheumatoid arthritis (RA)
- Ankylosing spondylitis
- Acute gouty arthritis
- Moderate pain associated with dental surgery
Mode of Action
- Potent, orally active cyclooxygenase-2 (COX-2) specific inhibitor
- Inhibition of COX-2 decreases pain, inflammation, and fever
- Selective inhibition of COX-2 decreases potential for GI toxicity and effects on platelet aggregation
Dosage & Administration
- Recommended doses for different conditions: 30mg-120mg once daily
- Short-term treatment for dental surgery pain
- Consideration for cardiovascular risks and lowest effective dose
Interaction
- Interaction with oral anticoagulants, diuretics, ACE inhibitors, angiotensin II antagonists, acetylsalicylic acid, ciclosporin, tacrolimus, lithium
- Take without regards to meals
Contraindications
- Hypersensitivity to the active substance or excipients
- Active peptic ulceration or GI bleeding
- Pregnancy and lactation
- Severe hepatic dysfunction, estimated renal creatinine clearance <30ml/min
- Inflammatory bowel disease, congestive heart failure, hypertension, established cardiovascular diseases
Side Effects
- Palpitation, fatigue, influenza-like symptoms, ecchymosis
- Dry mouth, taste disturbance, mouth ulcer, appetite and weight change
- Atrial fibrillation, transient ischaemic attack, chest pain, flushing
- Cough, dyspnoea, epistaxis, anxiety, impaired mental acuity
- Electrolyte disturbance, myalgia, arthralgia
- Rarely confusion, hallucinations
Pregnancy & Lactation
- Not recommended in women attempting to conceive
- Excreted in the milk of lactating rats
- Breastfeeding not recommended
Precautions & Warnings
- Caution in elderly, patients with prior GI disease, cardiovascular risk factors
- Consideration for renal function, cardiac failure, left ventricular dysfunction, hypertension
- Monitoring for liver function, hypersensitivity, fever and inflammation
- Co-administration with warfarin or oral anticoagulants
Overdose Effects
- No significant toxicity observed with single doses up to 500mg and multiple doses up to 150mg/day
- Usual supportive measures recommended in case of overdose
