Indications
- Osteoarthritis (OA)
- Rheumatoid arthritis (RA)
- Ankylosing spondylitis
- Acute gouty arthritis
- Moderate pain associated with dental surgery
Pharmacology
- Potent, orally active cyclooxygenase-2 (COX-2) specific inhibitor
- Selectively inhibits COX-2 without inhibition of COX-1
- Decreases clinical signs and symptoms with decreased potential for GI toxicity and effects on platelet aggregation
Dosage and administration
- 30 mg once daily for Osteoarthritis
- 60 mg once daily for insufficient relief from Osteoarthritis symptoms
- 90 mg once daily for Rheumatoid arthritis
- 90 mg once daily for Ankylosing spondylitis
- 120 mg once daily for Acute gouty arthritis
- 90 mg once daily for moderate pain associated with dental surgery
- Limited to maximum of 3 days for postoperative dental surgery pain
Interactions
- Increase in prothrombin time with oral anticoagulants
- May reduce the effect of diuretics and other antihypertensive drugs
- Can be used concomitantly with low-dose acetylsalicylic acid for cardiovascular prophylaxis
- May increase the nephrotoxic effect of ciclosporin or tacrolimus
- Decreases lithium renal excretion and increases plasma levels
Contraindications
- Hypersensitivity to the active substance or excipients
- Active peptic ulceration or gastrointestinal (GI) bleeding
- Experienced bronchospasm or allergic-type reactions after taking acetylsalicylic acid or NSAIDs
- Pregnancy and lactation
- Severe hepatic dysfunction
- Estimated renal creatinine clearance <30 ml/min
- Inflammatory bowel disease
- Congestive heart failure (NYHA ll-IV)
- Uncontrolled hypertension
- Established ischaemic heart disease, peripheral arterial disease, and/or cerebrovascular disease

Pregnancy and lactation
- Not recommended in women attempting to conceive
- Excreted in the milk of lactating rats
- Women who use it must not breastfeed
Precautions and warnings
- Caution advised with patients at risk of developing gastrointestinal complications with NSAIDs
- Exercise caution with patients at significant risk factors for cardiovascular events
- May cause reduction in prostaglandin formation and impair renal function
- Caution in patients with history of cardiac failure, left ventricular dysfunction, hypertension, pre-existing edema
- Monitor patients with symptoms suggesting liver dysfunction
- Discontinue at first appearance of hypersensitivity signs
- May mask fever and other signs of inflammation
- Exercise caution when co-administering with warfarin or other oral anticoagulants
Overdose effects
- Did not result in significant toxicity up to 500 mg single dose or 150 mg/day for 21 days
- Usual supportive measures should be employed
