Indications
- Symptomatic relief of osteoarthritis
- Symptomatic relief of rheumatoid arthritis
- Symptomatic relief of ankylosing spondylitis

Pharmacology
- Orally active COX-2 specific inhibitor
- Inhibition of prostaglandin-mediated pain, inflammation, and fever
- Decreased potential for gastrointestinal toxicity and effects on platelet aggregation
Dosage & Administration
- Recommended doses for osteoarthritis, rheumatoid arthritis, ankylosing spondylitis, acute gouty arthritis, and postoperative dental surgery pain
- Consideration of cardiovascular risks with dose and duration of exposure
Interaction with Medicine
- Effect on prothrombin time with oral anticoagulants
- Reduction of diuretic and antihypertensive drug effects by NSAIDs
- Concomitant use with acetylsalicylic acid for cardiovascular prophylaxis
- Potential nephrotoxic effect of coadministration with ciclosporin or tacrolimus
- Decreased lithium renal excretion and increased lithium plasma levels with NSAIDs
Contraindications
- Hypersensitivity to active substance or excipients
- Active peptic ulceration or gastrointestinal bleeding
- Bronchospasm, rhinitis, nasal polyps, allergic reactions to acetylsalicylic acid or NSAIDs including COX-2 inhibitors
- Pregnancy and lactation
- Severe hepatic dysfunction or estimated renal creatinine clearance <30 ml/min
- Children and adolescents under 16 years of age
- Inflammatory bowel disease, congestive heart failure, uncontrolled hypertension, established cardiovascular diseases
Side Effects
- Palpitation, fatigue, influenza-like symptoms, ecchymosis
- Dry mouth, taste disturbance, mouth ulcer, appetite and weight change
- Atrial fibrillation, transient ischaemic attack, chest pain, flushing
- Cough, dyspnoea, epistaxis, anxiety, impaired mental acuity
- Paraesthesia, electrolyte disturbance, myalgia, arthralgia
- Confusion and hallucinations (very rare)
Pregnancy & Lactation
- Not recommended in women attempting to conceive
- Excretion in lactating rats, not recommended for breastfeeding women
Precautions & Warnings
- Caution in patients at risk of gastrointestinal complication with NSAIDs
- Careful consideration in patients with significant risk factors for cardiovascular events
- Potential reduction in prostaglandin formation and renal blood flow, necessitating renal function monitoring
- Exercise caution in patients with cardiac failure, left ventricular dysfunction, hypertension, and pre-existing edema
- Monitoring of liver function, discontinuation at first sign of skin rash or hypersensitivity
- Caution in co-administration with warfarin or other oral anticoagulants
Overdose Effects
- No significant toxicity observed with single or multiple doses up to 150 mg/day
- Usual supportive measures in case of overdose
