Indications
- Symptomatic relief of Osteoarthritis (OA)
- Symptomatic relief of Rheumatoid arthritis (RA)
- Symptomatic relief of Ankylosing spondylitis
- Symptomatic relief of acute gouty arthritis
- Short-term treatment of moderate pain associated with dental surgery
Pharmacological Actions
- Potent COX-2 specific inhibitor
- Inhibition of prostaglandin-mediated pain, inflammation, and fever
- Decreases signs and symptoms of inflammation
- Selective inhibition of COX-2 within clinical dose range
- Decreased potential for gastric toxicity and effects on platelet aggregation
Dosage and Administration
- Recommended dose for Osteoarthritis: 30 mg once daily
- Recommended dose for Rheumatoid arthritis: 90 mg once daily
- Recommended dose for Ankylosing spondylitis: 90 mg once daily
- Recommended dose for Acute gouty arthritis: 120 mg once daily (for 8 days)
- Recommended dose for Postoperative dental surgery pain: 90 mg once daily (up to 3 days)
Interactions
- Increase in prothrombin time with oral anticoagulants
- Potential reduction in effect of diuretics and antihypertensive drugs
- Concomitant use with low-dose acetylsalicylic acid
- Potential increase in nephrotoxic effect when coadministered with ciclosporin or tacrolimus
- Decreased lithium renal excretion leading to increased lithium plasma levels
Contraindications
- Hypersensitivity to the active substance or excipients
- Active peptic ulceration or gastrointestinal (GI) bleeding
- History of allergic-type reactions to acetylsalicylic acid or NSAIDs
- Pregnancy and lactation
- Severe hepatic dysfunction
- Estimated renal creatinine clearance <30 ml/min
- Children and adolescents under 16 years of age
- Inflammatory bowel disease
- Congestive heart failure (NYHA ll-IV)
- Persistently elevated blood pressure above 140/90 mmHg
- Established ischaemic heart disease, peripheral arterial disease, and/or cerebrovascular disease

- Caution in patients at risk of gastrointestinal complications with NSAIDs
- Careful consideration for patients with significant cardiovascular risk factors
- Monitor renal function in patients with potential impairment
- Exercise caution in patients with cardiac failure, left ventricular dysfunction, or hypertension
- Monitor for signs of hepatic insufficiency
- Discontinue at the first appearance of hypersensitivity signs
- May mask fever and other signs of inflammation
- Exercise caution when co-administering with oral anticoagulants
Overdose Effects
- No significant toxicity observed with single doses up to 500 mg and multiple doses up to 150 mg/day for 21 days
- Employ usual supportive measures in case of overdose
